Facility Inspections

Grade: A-plus

This nursing home earned a solid overall grade as described above, but we really wanted to point out its nearly flawless inspections in recent years. We awarded them an impressive grade of A+ in this category. Inspection grades take several factors into consideration, including deficiencies and substantiated complaints. You can find more information about each of these issues by obtaining copies of nursing homes' inspection reports. While this nursing home had a few deficiencies on its report, none were major deficiencies based on CMS' scale. Keep in mind that deficiency-free inspection reports are rare in the industry.

Long-term Care Quality

Grade: B-plus

We also gave this nursing home a favorable score in long-term care. This facility received a B+ in this category, which happens to be one of our higher scores. For patients in need of a permanent place to live rather than rehabilitation, long-term care is a very important category. After assessing the quantity of care provided by aids and other staff, we then analyzed the facility's vaccination record. This nursing home vaccinated 77.13004 percent of its residents against pneumonia, which is a few points lower than we were hoping for but still a reasonably acceptable percentage. While its vaccination rate was a bit lower than we had hoped, we were pleased to find nursing home was able to limit hospitalizations. It had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low number.

Short-term Care Quality

Grade: D

Unfortunately, this facility's next best score is short-term care. However, with a D in this category, it still performed relatively poorly. In the area of short-term care, we try to evaluate indicators of a nursing home's rehabilitation services. We look at a nursing home's skilled nursing services, including the ones performed by registered nurses and physical therapists. In this nursing home's case, we found that it provides less physical therapist and registered nurse hours per resident than the average facility. Finally, we assessed the number of patients who were able to return to the community from this nursing home. This definitely was not a strength for this nursing home. We found that just 5.3 percent of this facility's residents returned home. At most facilities, around half of their patients are able to return home so this is well below average.

The last category we assessed is nursing. It received an abysmal F for this area, which is a poor score. This is certainly a significant concern. In determining a nursing home's nursing grade, we weigh the number of hours nurses are with residents and the levels of training of those nurses. This facility provided just 0.4 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also assessed some nursing quality-based metrics in determining our nursing grades. This facility performed well in terms of avoiding major falls and pressure ulcers.
