Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly impressive government inspection report this year. As a result, it received one of our more favorable scores in that area with a B. We take several aspects of a facility's inspection report into consideration in determining these inspection grades. One critical factor is health deficiencies. It should be noted that the severity of these deficiencies is arguably more important than the number of deficiencies, as some are quite minor. This particular nursing home received 6 deficiencies on its inspection report. Overall, this is still a solid inspection report, but we'd always recommend looking into any deficiencies you find troubling.
Nurse Quality
This facility's second best category was nursing, but even there it received a grade of just D. This is still a well below average grade. Nursing ratings are based in large part on levels of nurse staffing. This facility offered a meager 0.3 hours of nursing care per resident daily. This figure is significantly below the national average. Lastly, we also assessed some nursing quality-based metrics in calculating our nursing scores. This nursing home fared well in the area of avoiding major falls and pressure ulcers.

Short-term Care Quality
This nursing home's second least favorable score is short-term care, where it earned just a D in that area. This is a relatively poor grade. Short-term care grades are often used to measure a facility's performance with rehabilitation To have highly graded rehabilitation services, facilities generally must provide better levels of skilled nursing. These types of services include registered nurses, physical therapists, speech therapists and other licensed professionals. Given its weak score in this area, we were not stunned to learn this facility provided significantly fewer registered nurse and physical therapist hours per patient than most other nursing homes. Lastly, we considered the percentage of residents who ultimately were able to return home from this facility. This nursing home didn't fare well here either. In fact, we found that just 4.2 percent of this facility's patients were able to return home.
Long-term Care Quality
The last area we analyzed is long-term care, in which this nursing home received a poor grade in this category as well. It received an abysmal F in this area, which is obviously a major disappointment. Nursing homes that do not fare well in this category typically are not as well-staffed and are lagging in a few of the areas of routine healthcare services we looked at. On top of assessing the amount of care provided by aids and other staff, we analyzed the number of residents vaccinated for pneumonia. This facility vaccinated 92.22973 percent of its patients, which is slightly less than we anticipated. To our surprise, this place actually fared well at keeping its residents out of the hospital. It had only 0 hospitalizations per 1,000 long-term resident days. This was its best score in this category.
