Facility Inspections
Unfortunately, we did not have any good things to write about this facility's overall grade or its category grades. Its best category is the area of inspections. Nevertheless, with a D in this area, it still performed relatively poorly. Our inspection ratings weigh a host of factors included in a nursing home's inspection report. Generally, you should be most cautious when considering a nursing home that has severe deficiencies on a recent inspection report. This nursing home had a category G through L deficiency, which rank among the more severe deficiencies. These categories indicate that the deficiencies found by CMS had the potential to pose actual harm to patients. Finally, we also want to point out that this place was assessed large government fines of more than $50,000. This often signifies that government inspectors grew tired after several years of bad inspections.
Nurse Quality
One of the other reasons we rated this nursing home so highly is that it received a very impressive nursing score. In that area, we awarded this nursing home an F. Nursing grades are mostly associated with the facility's nurse staffing. With just 0.3 hours of nursing care per patient per day, this facility's nurse staffing levels were well below the national average. Finally, we also assessed some nursing quality measures in calculating our nursing grades. This place fared well in terms of minimizing its residents' major falls and pressure ulcers.

Long-term Care Quality
This facility wound up receiving a an abysmal score in our long-term care category. It received an F in this category. If you are looking for anything other than rehabilitation, you should take a close look at long-term care scores. In addition to assessing the quantity of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This facility administered the vaccine to 95.07693 percent of its residents. To our surprise, this nursing home was able to limit hospitalizations. With only 0.01 hospitalizations per 1,000 long-term resident days, this nursing home had less hospitalizations than the average nursing home. Sadly, a few of its other scores in the category weren't as favorable as these.
Short-term Care Quality
The last area we looked at is short-term care. This nursing home was given an abysmal grade of F in this category. In our short-term care score, we endeavor to craft a fair barometer for rehabilitation services. In doing so, we look at a facility's offerings of skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Considering its score in this category, we were not surprised to find that this nursing home is well below average in terms of the quantity of physical therapy and registered nurse hours offered to its residents based on the metrics we assessed. Finally, we looked at the number of residents who eventually returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 3.6 percent of this facility's patients were able to return home.
