Registered Staff Hours
While we did not rate this nursing home well overall, we would like to point out its decent government inspection reports in recent years. We gave it a relatively respectable score of B- in that area. We believe that one of the most critical factors in evaluating an inspection report is deficiencies. We especially focus on the severity of the deficiencies. Fortunately, although this facility had some minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G through L. This means that CMS didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
This facility's second highest category ended up being long-term care. In that category, we gave this nursing home a B-. For patients looking for a permanent place to live rather than rehabilitation, long-term care grades are a key measure. Once we looked at the volume of care provided by aids and other staff, we then analyzed the nursing home's vaccination data. We were pleased to learn that this nursing home vaccinated 100 percent of its residents for pneumonia. Finally, this place was also able to limit hospitalizations. In fact, it had less than one hospitalization per one thousand long-term resident days, which is a remarkably low figure.
Does this page answer your questions about Clearstream Rehab & Nursing Center?
We awarded this facility a respectable grade in the area of nursing as well, with a grade of B-. There are a number of criteria included in this area. Most of these criteria reflect levels of nurse staffing. This nursing home provides 0.4 hours of nursing care per resident each day. This is a much lower figure than we are used to seeing. Despite having low quantities of nursing care, this place actually excelled in some of the quality measures we looked at. It performed well when it comes to avoiding major falls and pressure ulcers.
Short-term Care Quality
The final category we assessed is short-term care. This facility was given a rock bottom grade of F in this category. Short-term care grades are vital for individuals looking for rehabilitation. Rehabilitation generally requires additional skilled nursing services. Skilled nursing means a wide range of services, spanning from registered nurses to physical and speech therapists, as well as other types of therapy. Based on its grade in this category, we weren't surprised to find that this nursing home was well below average in terms of the quantity of physical therapy and registered nurse hours offered to its patients based on the measures we assessed. Honestly, this was about what we expected here. Finally, we looked at the number of patients that eventually returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 4.4 percent of this facility's residents were able to return home. This figure was well off the national average.
