Long-term Care Quality
To go along with its strong overall score, we gave this nursing home a grade of A for our long-term care rating. Nursing homes that receive this type of grade in long-term care tend to provide consistent around the clock care to ensure residents are kept in good health. One of the factors we considered on top of nursing hours is vaccines. This nursing home provided the pneumonia vaccination to 100 percent of its residents. Vaccination is a proven method to minimize unnecessary deaths and hospitalizations for the senior population. Finally, this place was able to limit hospitalizations. Indeed, it had less than one hospitalization per one thousand long-term resident days, which is an impressively low figure.
Facility Inspections
This facility also excelled in inspections. We gave it an A- for that category. We weigh several aspects of a nursing home's inspection report in calculating our inspection ratings. One of those factors is deficiencies. It should be noted that the severity of deficiencies is arguably more important than the quantity of deficiencies, as some deficiencies end up being quite insignificant. Fortunately, although this nursing home had a few minor dings on its report, it had zero severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This tells you that CMS did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Nurse Quality
Turning to the category of nursing care, this facility didn't perform quite as well in this area as some of the categories discussed above. Nevertheless, a grade of B- in this category is by no means a bad score. In calculating a nursing home's nursing score, we assess the quantity of hours nurses spend with residents and the levels of training of the nurses. This place provides only 0.3 hours of nursing care per patient each day. This is a somewhat alarming figure which is well below the national average. Despite having low quantities of nursing care, this nursing home fared better in some of the quality-based metrics we assessed. In fact, it performed well in the area of minimizing its residents' pressure ulcers and major falls. We consider these areas to be good indicators of the quality of nursing care. In some cases, the quality of nursing care is every bit as important as the number of hours of care provided.
Short-term Care Quality
This facility's least impressive area is short-term care, which is the last category we looked at. For this category, we gave this facility a grade of just D. Short-term care grades are often employed to gauge a facility's performance with rehabilitation. To offer high-end rehabilitation services, facilities generally need to provide higher levels of skilled nursing services. Skilled nursing includes registered nurses, physical therapists, occupational therapists and other highly trained professionals. Unfortunately, we found that this nursing home provided less registered nurse and physical therapist hours per patient than most nursing homes. The final item we assessed in this area is the number of residents who were able to leave the facility and return to the community. We discovered that just 4.3 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar metrics doomed this facility's short-term care score.
