Facility Inspections

Grade: A-plus

The primary reason this ended up being a decent nursing home is that it received an elite inspection grade. Its inspection grade was far superior to its overall score. In our inspections category, we gave this facility an A+. We weigh several aspects of a nursing home's inspection report in calculating these inspection scores. One of those factors is deficiencies. It should be noted that the severity of the deficiencies is arguably more important than the number of deficiencies, as some deficiencies end up being quite minor. Fortunately, although this facility had a few minor dings on its report, it had zero severe deficiencies. Severe deficiencies are ones found in categories G through L. This means CMS didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is always a good sign.

Long-term Care Quality

Grade: B-plus

This facility also was awarded a strong long-term care grade. As a matter of fact, we gave it a grade of B+ in that category, which happens to be one of our more favorable scores. If you are seeking anything other than rehabilitation, you should take a close look at long-term care ratings. On top of looking at the volume of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This facility gave the vaccine to 86.56716 percent of its patients. This is a bit below what we expected. While its vaccination rate was a bit lower than we hoped for, this facility was able to limit hospitalizations. In fact, it had less than one hospitalization per one thousand long-term resident days, which is a remarkably low number.

Short-term Care Quality

Grade: D

This nursing home's second least favorable score is short-term care, where it received a grade of just D in that area. This is a well below average score. In the category of short-term care, we try to evaluate indicators of a nursing home's rehabilitation services. We assess the nursing home's skilled nursing services, including those performed by registered nurses and physical, occupational and other therapists. Given its weak grade in this area, we were not stunned to discover this nursing home provides substantially fewer registered nurse and physical therapist hours per resident than most facilities. The last item we assessed in this category is the number of residents who were able to leave the facility and return to the community. We discovered that just 0 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar statistics doomed this nursing home's short-term care grade.

The last area we assessed is nursing, which was this nursing home's worst category. We gave it a lowly F in this category, which is certainly very concerning. Our nursing rating weighs several subcategories. The most heavily weighted factor is the number of hours nurses spent with residents. This nursing home offered a meager 0.3 hours of nursing care per resident per day. Lastly, we also assessed some nursing quality-based metrics in calculating our nursing scores. This nursing home fared well in the area of minimizing its residents' pressure ulcers and major falls.
