To go along with its decent overall score, we awarded this nursing home a grade of A- for our nursing rating. This score is far more impressive than the facility's overall grade. Our nursing grade features numerous subcategories. The most heavily weighted one is the amount of time nurses spent with residents. This particular facility provided 4.2 hours of nursing care per resident daily, which was among the highest figures we found. Lastly, this facility also performed well in several of the quality-based metrics we looked at. For example, in terms of the percentage of its patients sustaining falls leading to major injury, this facility performed as well as any nursing home in the state. Avoiding major falls is typically a good indicator that a place has reliable quality controls in place. Major falls can many times be prevented if more nurses aids and better safety protocols are in place.

Inspections is another area where this nursing home excelled. In fact, we awarded this facility a grade of B+ for our inspections rating. This score is far more impressive than its overall grade. Perhaps the most important factor we consider in computing our inspection scores is deficiencies. Deficiencies can be found on a facility's inspection reports. Places with better grades in this category typically dodged the most severe deficiencies involving patient abuse or death. This facility was assessed 7 deficiencies on its inspection report, but none were considered to be severe deficiencies. This means that CMS didn't consider any of the deficiencies to create an imminent risk to patient safety or health. The fact that the deficiencies were relatively minor made us feel better about this inspection report.

We also gave this facility an impressive grade in short-term care. This facility earned a grade of B in this area, which is one of our more favorable scores. Short-term care scores are critical for prospective residents requiring rehabilitation. Rehabilitation generally utilizes higher levels of skilled nursing. Skilled nursing includes a vast scope of services, ranging from registered nurses to physical and occupational therapists, as well as other types of therapy. One of this nursing home's strengths is physical therapy hours. We found that it offers more hours with physical therapists to its residents than most facilities. Finally, we looked at the percentage of patients who were able to return to the community from this nursing home. We found that 33.2 percent of this facility's patients returned home.

The final area we rated was long-term care, proved to be this facility's least impressive category. We awarded this nursing home a grade of D in that category. This is one of the few weak links in an otherwise strong profile. In a long-term care environment, the facility's primary goal is to maintain patients' quality of life and keep them safe. In addition to offering favorable levels of nursing care, this nursing home provided the pneumonia vaccine to 100 percent of its patients. These marks are obviously more favorable than some of its other marks in the category. Surprisingly, this place was able to limit hospitalizations. With just 0 hospitalizations per 1,000 long-term resident days, this nursing home had fewer hospitalizations than the average nursing home. Unfortunately, some of its other scores in this category weren't as favorable as these.
