In addition to being a quality facility overall, this facility really excelled at short-term care, where it received an A. With our short-term care rating, we strive to forge a fair measure for rehabilitation. In doing so, we look at a nursing home's offerings of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. One of this nursing home's strengths is physical therapy hours. We found that it offers more hours with physical therapists to its residents than most facilities. The last metric we looked at in this area is the percentage of residents who were able to leave the nursing home and return to the community. We found that it fared as well as just about any nursing home in Indiana in this area with 64.2 percent of its patients returning home.

In addition, this facility also earned excellent inspections in recent years. We gave them one of our better scores in that category, with a grade of A. Inspection grades are based on many datapoints located in the a nursing home's inspection reports. Facilities that receive favorable grades in this category have few deficiencies on those reports. Most importantly, these places should not have any severe deficiencies which are associated with endangerment of patients. Although this facility had some deficiencies on its government inspection report, none were serious based on CMS' scale. The fact that the deficiencies were relatively minor made us feel better about this inspection report.

This nursing home's next best category was nursing. We awarded it a grade of B- in this category. While this wasn't quite as good as some of its other category grades, this isn't a bad score either. The nursing grade is based on a host of subcategories. The most heavily weighted factor is the number of hours nurses spend with patients. This facility provided 4.5 hours of nursing care per resident per day. This is an impressive figure. Lastly, we also factored a few quality measures into our nursing grades. This facility was relatively weak in two of the major areas we considered, with below average marks for minimizing its patients' major falls and pressure ulcers.

This combination proved to be effective as this facility was able to keep its residents out of the hospital.
