Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection grades are tied to several pieces of information found on the a nursing home's recent inspection reports. Deficiencies are a key item to look for on these reports. Severe deficiencies indicate a threat to the well being of residents. While this nursing home had some deficiencies on its inspection report, none of them were serious based on CMS' deficiency scale. Finally, CMS cited this nursing home for possible abuse or neglect. Hopefully, its performance improves in the future, but until then we simply can't recommend this facility.

This facility really excelled in our nursing category, where it earned an D. We analyzed the levels of nurses at the nursing home, as well as the number of hours the nurses were with residents, in determining our grade in this category. This facility offered just 0.3 hours of nursing care per patient per day. This number is far below average. Finally, we also assessed some nursing quality-based metrics in determining our nursing scores. This facility performed well in terms of avoiding major falls and pressure ulcers.

Turning to the category of long-term care, this facility was given a grade of just D in this category. In a long-term care environment, the facility's primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. On top of considering the quantity of care provided by nurses aids and other staff, we looked at the percentage of residents vaccinated for pneumonia. This nursing home gave the vaccine to 98.69281 percent of its patients. To our surprise, this facility also keeps its residents out of the hospital. It had just 0.01 hospitalizations per 1,000 long-term resident days. Sadly, a few of its other scores in the category weren't as favorable as these.

Moving on to our fourth area, this nursing home really didn't fare well in this area either. Due to a very poor grade of F in short-term care, this is really as terrible as it gets. Our short-term care ratings are considered to be most important for patients requiring a nursing home for rehabilitation. Rehabilitation typically utilizes more highly-skilled nursing. This means not only nursing, but also physical and respiratory therapy, as well as other types of therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last datapoint we assessed in this category is the percentage of residents who were able to leave the facility and return to the community. We discovered that just 4.2 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics was damning for this facility's short-term care grade.
