Long-term Care Quality

Grade: A-minus

In addition to performing well in its overall grade, this facility received an excellent long-term care grade. In fact, we gave it a grade of A- in that category. In a long-term care setting, the facility's primary goal is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. On top of assessing the amount of care provided by nurses aids and other staff, we analyzed the percentage of residents vaccinated for pneumonia. This facility gave the vaccine to 74.57627 percent of its residents. This is slightly lower than we expected. Clearly, this facility is doing something right in this area as it also excelled at keeping its residents out of the hospital. It had only 0.02 hospitalizations per one thousand long-term resident days, which is an impressively low figure.

Facility Inspections

Grade: A-minus

This nursing home has received near flawless inspections recently. We awarded them an A- in this area. Inspection grades take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can find more information about each of these factors by obtaining copies of nursing homes' inspection reports. This place was hit with 6 deficiencies on its inspection report, but fortunately none of the deficiencies were considered to be severe. This means that CMS did not consider any of the deficiencies to create an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Nurse Quality

Grade: B-plus

In addition, this facility also received favorable nursing grades this year. In fact, we awarded them one of our better scores in that area, with a grade of B+. Our nursing score is based on the nursing home's nurse staffing levels. We weigh both the levels of skill of the nurses as well as the number of hours spent with patients. This particular nursing home provided 0.3 hours of nursing care per resident per day. Finally, we also assessed several nursing quality-based metrics and this place excelled in some of these areas. With less than five percent of its residents sustaining pressure ulcers, it performed as well as any place Nebraska in this category. This is generally a good indicator of quality nursing care. Pressure ulcers can many times be prevented by providing better nursing care, such as having a system of turning residents more frequently.

Short-term Care Quality

Grade: B

The last category we looked at was short-term care, where this facility was given a grade of B. This actually proved to be this nursing home's least impressive area. A facility could be doing a lot worse when their worst score still beats out most other nursing homes. In crafting our short-term care grades, we look at the facility's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our purpose is to devise a barometer for sizing up the rehabilitation services of various nursing homes. Fortunately, it appear that this facility employs registered nurses. Not every facility employs these types of nurses. However, according to the information this nursing home provided, it does not look like the facility employs physical therapists. The last datapoint we considered in this category is the percentage of residents who were able to leave the nursing home and return to the community. We found that 4 percent of this nursing home's patients returned home.
