Facility Inspections
Although we gave this facility a poor overall grade, we gave it a B for our inspections rating. This is due to the nursing home receiving a quality inspection report this year. Perhaps the most significant factor we look at in determining our inspection grades is deficiencies. These deficiencies can be found on a nursing home's recent inspection reports. It is generally in your best interest to avoid places that had too many severe deficiencies. This facility was assessed 4 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered to be major deficiencies. This means that CMS did not consider any of these deficiencies to pose an immediate risk to resident health or safety. A few minor deficiencies are not the end of the world.
Short-term Care Quality
We were also pleasantly surprised to find that this nursing home earned a decent short-term care grade this year. We gave it a C in this category. In calculating our short-term care ratings, we size up the nursing home's levels of skilled nursing, which includes registered nurses, physical therapists occupational therapists and other types of therapists. This grade is considered to be a fair measure of the nursing home's rehabilitation. We were surprised to find that this facility was below average in the two key staffing areas we looked at. The facility offered fewer registered nurse and physical therapist hours per resident than most other facilities. The final metric we assessed in this area is the percentage of patients who eventually returned home from the nursing home. We found that just 6.1 percent of this nursing home's patients returned home. This figure was below average.

Long-term Care Quality
This facility's second least favorable category was long-term care, where it earned just a D in that category. This is a well below average grade. For long-term care residents, the primary objective is to maintain patients' quality of life and keep them safe. On top of considering the quantity of care provided by aids and other staff, we analyzed the percentage of residents vaccinated for pneumonia. This facility vaccinated 99.5984 percent of its patients. Surprisingly, this facility was able to keep its residents out of the hospital. In fact, it had just 0.01 hospitalizations per one thousand long-term resident days. Sadly, a few of its other scores in this category weren't as favorable as these.
Nurse Quality
The last category we graded is nursing, in which this facility received a very poor score here also. We gave it an F in this area, which is obviously a major disappointment. Our nursing rating analyzes the nursing home's nurse staffing levels. We weigh both the levels of skill of those nurses and the amount of time spent with patients. This facility offered only 0.2 hours of nursing care per resident daily. This is not a very impressive figure. Finally, we also looked at some nursing quality-based metrics in determining our nursing scores. This nursing home performed well in terms of avoiding major falls and pressure ulcers.
