Facility Inspections
While we graded this nursing home incredibly poorly overall, it earned a very impressive inspection report this year. As a result, it received one of our best grades in that category with an A. Arguably the most important factor we consider in determining our inspection grades is deficiencies. These deficiencies are found on a facility's inspection reports. It is typically in your best interest to avoid nursing homes that have too many deficiencies. While this nursing home had some minor dings on its government inspection report, it had no severe deficiencies. Severe deficiencies are labeled as categories G, H, I, J, K and L. This tells you the government inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not the end of the world.
Short-term Care Quality
In our next category, we gave this nursing home a rock-bottom F in the area of short-term care. In calculating our short-term care ratings, we analyze the nursing home's levels of highly skilled nursing services, which includes registered nurses, physical therapists speech therapists and other licensed professionals. This category is generally a fair measure of a nursing home's rehabilitation services. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we considered the percentage of residents who were able to return to the community from this facility. This was not a strength for this nursing home. We found that just 0 percent of this nursing home's patients returned home. At most facilities, around half of their residents are able to return home so this is well below average.

Nurse Quality
Unfortunately, we gave this facility an F for its nursing grade. The nursing score weighs quite a few components, most of which are tied to levels of nurse staffing. This facility averaged just 0.3 hours of nursing care per resident per day. This is well below average. Lastly, we also looked at some nursing quality-based metrics in calculating our nursing scores. This place performed well in terms of avoiding major falls and pressure ulcers.
Long-term Care Quality
The last category we analyzed is long-term care. Unfortunately, we gave it an abysmal F for this category, which is a bottom of the barrel score. When nursing homes receive a score in this range in this category it generally means the nursing home didn't perform well in our quality measures relating to patient care. One of the criteria we considered after nurse's aid hours was vaccines. Candidly, we were a bit worried that this nursing home provided the pneumonia vaccine to only 72.22222 percent of its patients. Surprisingly, this facility actually fared well at keeping its residents out of the hospital. It had only 0.01 hospitalizations per one thousand long-term resident days. This was its best feature in this category.
