Facility Inspections
Although we did not rate this nursing home highly overall, we want to draw your attention to its excellent inspections in recent years. We awarded them an A in this category. Inspection grades are tied to several items found on the a nursing home's recent inspection reports. Deficiencies are an important item to look for on these reports. Most importantly, you should avoid nursing homes with severe deficiencies associated with risks to resident well being. Although this place had some deficiencies on its government inspection report, none were major deficiencies based on CMS' scale. A couple minor deficiencies aren't necessarily the end of the world.
Short-term Care Quality
Turning to the category of short-term care, this facility received just a D in that area. Sadly, this proved to be one of its best areas. In the area of short-term care, we try to evaluate measures of a facility's rehabilitation services. We assess a facility's skilled nursing services, such as the ones performed by registered nurses and physical therapists. With its score in this category, we were not stunned to find that this nursing home is well below average in terms of its number of physical therapy and registered nurse hours provided to its residents based on the measures we assessed. Frankly, this was what we expected to find. Lastly, we assessed the percentage of patients who were able to return to the community from this facility. This definitely wasn't a strength for this nursing home. We found that just 0 percent of this nursing home's residents returned home. At most facilities, around half of their residents are able to return home so this is well below average.

Long-term Care Quality
We gave this nursing home a grade of just D for our long-term care rating. This is not a very good score. When nursing homes receive a grade in this range in long-term care it generally means the facility didn't perform well in our measures relating to resident care. Once we finished looking at the amount of nursing care, we next considered the nursing home's vaccination records. Frankly, we were a bit bothered by the fact that this facility vaccinated a relatively low 64.575645 percent of its patients against pneumonia. Surprisingly, this place was actually able to limit hospitalizations. With just 0 hospitalizations per 1,000 long-term resident days, this nursing home has fewer hospitalizations than many nursing homes. This is its best score in this category.
Nurse Quality
This facility's worst category is nursing, which is the last category we scored. For this category, we awarded this nursing home a grade of D. Our nursing category weighs several factors, most of which are tied to nursing hours. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Lastly, we also looked at some nursing quality measures in calculating our nursing grades. This place fared well in the area of minimizing its residents' pressure ulcers and major falls. We consider these areas to be indicators of the quality of nursing care.
