Long-term Care Quality

Grade: C

Although we gave this facility a terrible overall grade, we awarded it a grade of C for our long-term care rating. Our long-term care grade assesses a facility's personal care rather than the skilled nursing and healthcare services that are critical to a rehabilitation facility. On top of looking at the volume of care provided by aids and other staff, we analyzed the percentage of residents vaccinated for pneumonia. Fortunately, this nursing home gave the vaccine to 99.3921 percent of its patients, which is an above average total. This facility keeps its residents out of the hospital. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is an impressively low number.

Nurse Quality

Grade: D

This facility's second best category was nursing, but even there it received a grade of just D. This is still a well below average grade. The nursing category consists of quite a few datapoints, many of which are associated with levels of nurse staffing. With just 0.3 hours of nursing care per resident each day, this facility's nurse staffing levels were well below average. Lastly, we also assessed some nursing quality-based metrics in determining our nursing ratings. This place performed well when it comes to minimizing its residents' pressure ulcers and major falls. We consider these areas to be indicators of the quality of nursing care.

Facility Inspections

Grade: F

Unfortunately, this facility received very poor government inspection reports in recent years, causing them to receive an F in our inspection category. An F in this area is generally a signal that there were red flags on the facility's government inspections. We would advise paying special attention to any deficiencies on a facility's inspection reports when it receives this poor of an inspection rating. We would pay close attention to the level of severity of the deficiencies. This particular nursing home was assessed one category G through L deficiency, which rank among the more serious categories of deficiencies. This generally indicates that the deficiencies uncovered by inspectors had the potential to cause actual harm to patients. Another area of concern is that CMS flagged this nursing home for possible abuse or neglect. This is not what you want to see. We advise you to direct your search elsewhere.

Short-term Care Quality

Grade: F

The last category we looked at is short-term care. This facility was given an abysmal grade of F in that category. In our short-term care category, we strive to forge a meaningful gauge for rehabilitation services. In this process, we analyze a nursing home's scope of skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last measure we assessed in this category is the number of residents that who were able to eventually return home from the nursing home. This facility struggled quite a bit in this area as well, with just 3.2 percent of its residents returning home.
