Facility Inspections

Grade: B-minus

Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection ratings weigh several factors, including deficiencies and substantiated complaints. You can find more information about each of these issues by reviewing copies of nursing homes' inspection reports. This particular nursing home received 5 deficiencies on its inspection report. In addition to having severe deficiencies, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.

Short-term Care Quality

Grade: B-minus

This facility also didn't rate poorly in the area of short-term care. In fact, we gave it a B- in that area, which ended up being another one of its more favorable scores. Short-term care grades are crucial for prospective residents in need of rehabilitation. Rehabilitation usually mandates higher levels of highly-skilled nursing services. Skilled nursing includes a wide range of services, spanning from registered nurses to physical and occupational therapists, as well as other forms of therapy. We found that this facility was below average in the two key staffing areas we assessed. In fact, it provided fewer physical therapist and registered nurse hours per resident than most other facilities. Finally, we considered the number of patients who returned home from this facility. We found that 6.5 percent of this facility's patients were able to return home. This figure was well below the national average.

Nurse Quality

Grade: D

Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. There are a host of criteria within this area. Many of the criteria are tied to the quantity and quality of nurse staffing. With just 0.3 hours of nursing care per patient daily, this place's nurse staffing levels were well below the national average. Finally, we also assessed some nursing quality measures in determining our nursing ratings. This facility fared well when it comes to avoiding major falls and pressure ulcers.

Long-term Care Quality

Grade: F

The final area we rated was long-term care. Sadly, it received an abysmal F in this category, which is a bottom of the barrel grade. For long-term care residents, the primary objective is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. One of the statistics we considered in addition to nursing hours was vaccinations. Fortunately, this nursing home administered the pneumonia vaccination to 94.12752 percent of its patients. To our surprise, this facility was able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this nursing home had fewer hospitalizations than the majority of nursing homes. Sadly, a few of its other scores in the category weren't as favorable as these.
