Facility Inspections

Grade: A-minus

While this facility's overall grade was as bad as it gets, it actually excelled in the category of inspections. In fact, we gave it an A- for that category, which is one of our highest scores. Arguably the most critical factor we look at in calculating our inspection ratings is deficiencies. Deficiencies can be found on a nursing home's inspection reports. It is typically best to avoid nursing homes that had a long list of deficiencies. This nursing home was assessed 2 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered severe. This means that the inspectors did not deem any of these deficiencies to pose an imminent risk to patient health or safety. A couple minor deficiencies aren't necessarily the end of the world.

Short-term Care Quality

Grade: D

Sadly, this nursing home's next most favorable score is short-term care. However, with a D in this area, it still performed relatively badly. Short-term care grades are generally used to grade a nursing home's rehabilitation services. In order to provide quality rehabilitation services, facilities generally must provide better levels of skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other licensed professionals. With a score in this range in this area, we were not shocked to find this place was below average in two of the key staffing metrics we looked at. It provided significantly fewer registered nurse and physical therapist hours per patient than most nursing homes. The last metric we assessed in this area is the number of residents who were able to leave the facility and return to the community. We discovered that just 0 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar metrics was damning for this facility's short-term care score.

Nurse Quality

Grade: D

Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. When calculating our nursing grades, we look at both staffing levels and the levels of licensure of those nurses. This nursing home provided only 0.2 hours of nursing care per resident daily. This is not a very impressive figure. Lastly, we also looked at some nursing quality measures in calculating our nursing scores. This nursing home fared well in terms of avoiding pressure ulcers and major falls. Many consider these datapoints to be indicators of the quality of nursing care.

Long-term Care Quality

Grade: F

The last category we rated was long-term care. It received an abysmal F in this area, which is a rock bottom score. Facilities that receive this kind of grade in this category may not provide the kind of consistent 24/7 care that some other nursing homes provide. After assessing the volume of care provided by nurses aids and other staff, we then analyzed the facility's vaccination record. Unfortunately, this facility vaccinated a relatively low 78.902954 percent of its residents for pneumonia. To our surprise, this place was actually decent at keeping its residents out of the hospital. It had only 0.02 hospitalizations per 1,000 long-term resident days. This was its best feature in this category.
