We gave this facility a grade of A in the short-term care category. In our short-term care grade, we seek to craft a sound measure for rehabilitation services. In this process, we assess a nursing home's offerings of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. We were pleased to find that this nursing home employs both registered nurses and physical therapists. The same can't be said for all nursing homes. The final item we looked at in this category is the percentage of residents that eventually returned home from the facility. We found that it fared as well as just about any nursing home in Oregon in this area with 83.7 percent of its residents returning home. For most facilities, fewer than half of their short-term residents are able to return home.

This nursing home performed well in nursing. In fact, we awarded it an B- for that category, which is one of our best grades. Nursing ratings are primarily tied to the nursing home's nurse staffing. With 5.9 hours of nursing care per resident per day, this nursing home surpassed the vast majority of facilities. Lastly, we also factored some quality measures into our nursing grades. Specifically, we looked at the percent of residents who sustained pressure ulcers and major falls. We view these statistics as predictive measures of the quality of nursing care, since better care tends to reduce these problems.

This facility was given its next most favorable category grade in the area of long-term care. We awarded this facility a grade of C in this category, which is an acceptable grade. In our long-term care rating, we analyze the volume of services provided by a facility, as well as a few quality statistics. This facility's vaccination statistics weren't as favorable as its number of nursing hours per resident. This facility gave the pneumonia vaccine to just 84.946236 percent of its patients. We'd love to see some improvement in this area next year. Nevertheless, this facility excelled at keeping its patients out of the hospital. Indeed, it had only 0 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.

Historical

Unfortunately, this nursing home did not receive a favorable inspection grade. We gave it a score of just D in this category. Inspection scores relate to a nursing home's government inspection reports. One of the most important factors in evaluating an inspection report is deficiencies. We recommend you scrutinize the severity of the deficiencies. This particular facility had 6 deficiencies on its inspection report. The only positive thing we found is that none of its deficiencies were in the categories that indicate that they caused a threat to resident safety or health. Finally, we want to draw your attention to the fact that this place received significant government fines recently of more than $50,000. This generally signifies that CMS inspectors grew tired after multiple years of bad inspections.
