Facility Inspections
Although this nursing home's overall score was as bad as it gets, it actually fared well in the category of inspections. In fact, we awarded it a B for that area, which is one of our better scores. Inspection grades account for several factors found on a nursing home's inspection report. One of the most important criteria we consider is the number and severity of deficiencies. You generally want to avoid places with a bunch of deficiencies flagged. This place was hit with 4 deficiencies on its inspection report, but none of the deficiencies were considered to be severe. This tells you that the inspectors didn't deem any of the deficiencies to pose an imminent threat to patient safety or health. A few minor deficiencies aren't the end of the world.
Nurse Quality
One of the other reasons we graded this nursing home so highly is that it earned a very impressive nursing score. Nursing turned out to be its second best category grade. In that area, we awarded this facility an D. Nursing grades are heavily correlated with quantity of nursing care available. This facility provided just 0.4 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also assessed some nursing quality measures in computing our nursing scores. This nursing home fared well in terms of avoiding major falls and pressure ulcers.

Long-term Care Quality
This facility's second least favorable category was long-term care, where it earned just a D in that category. This is a relatively poor grade. For prospective patients seeking a permanent place to live rather than rehabilitation, long-term care grades are an important measure. In addition to looking at the volume of care provided by aids and other staff, we analyzed the percentage of residents vaccinated for pneumonia. This facility administered the vaccine to 94.62366 percent of its residents, which is higher than the vast majority of nursing homes. Surprisingly, this facility was able to limit hospitalizations. With only 0.03 hospitalizations per 1,000 long-term resident days, this nursing home had fewer hospitalizations than many nursing homes. Sadly, some of its other scores in the category were not as favorable as these.
Short-term Care Quality
Switching gears to the last category, this facility really didn't fare very well in this area either. Due to a poor grade of F in short-term care, this is just about as as uninspiring as it gets. In forming our short-term care grades, we assess the facility's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our goal is to create a measure for sizing up the rehabilitation services of various facilities. With a grade this poor in this category, we were not surprised to discover that this nursing home was below average in the two key staffing areas we focus on. The nursing home offered far fewer registered nurse and physical therapist hours per patient than most other facilities. Finally, we looked at the percentage of residents who were able to return to the community from this nursing home. This definitely was not a strength for this nursing home. We found that just 4.2 percent of this nursing home's patients returned home. At most facilities, about half of their patients are able to return home so this is well below average.
