Short-term Care Quality
While we were not high on this facility's overall score, it actually received an impressive short-term care grade this year. We awarded it a B in this category, which proved to be a rare glimmer of hope in its profile. Our short-term care grades are meaningful for people needing rehabilitation. Rehabilitation typically mandates more highly-skilled nursing services. Skilled nursing means a broad spectrum of nursing services, spanning from registered nurses to physical and occupational therapists, as well as other forms of therapy. This nursing home provided more physical therapist and registered nurse hours per resident than most facilities. This is what we like to find when assessing a nursing home in this category. The final metric we assessed in this area is the percentage of residents who returned home from the nursing home. We found that 43.9 percent of this nursing home's patients returned home rather than remaining at the facility on a permanent basis.
Nurse Quality
We also ended up giving this facility a fairly respectable grade in the area of nursing, where it received a grade of B-. Nursing grades are based in large part on quality and quantity of nursing care. This particular nursing home provided 3.6 hours of nursing care per resident per day. Lastly, our nursing grades also factor in quality-based metrics, such as avoiding major falls. This place performed admirably in this statistic. Avoiding serious falls is generally a good indicator that a nursing home has quality controls in place. Many falls can be prevented if a nursing home provides enough nurses aids to assist its patients.

Facility Inspections
This nursing home earned mediocre inspections in recent years. We awarded them a C in this category. Our inspection grades are based on datapoints found on the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these reports. Severe deficiencies indicate a threat to patient safety. This place was assessed 14 deficiencies on its inspection report, but fortunately none were in category J, K or L. This indicates that CMS didn't consider any of the deficiencies to place resident safety or health in imminent harm or jeopardy. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.
Long-term Care Quality
The final category we analyzed was long-term care. This nursing home received a very poor grade of F in that category. If you are looking for anything other than short-term rehabilitation, you should take a close look at long-term care grades. Once we looked at the volume of care provided by nurses and other staff, we then analyzed the facility's vaccination statistics. This facility vaccinated 99.5935 percent of its residents for pneumonia, which is better than most nursing homes. To our surprise, this nursing home also was not as bad as we expected at keeping its patients out of the hospital. Although it had 2.28 hospitalizations per 1,000 long-term resident days, its risk adjusted score was not bad since it had more complex patients.
