Neither this facility's overall score nor its category grades gave us many positives to write about. This place's best category was inspections, but even there we gave it a D. This is still a very poor score. One of the most significant factors in assessing inspection reports is deficiencies. We suggest you pay close attention to the severity of the deficiencies. This nursing home had a category G through L deficiency, which are among the more serious deficiencies. These categories indicate that the deficiencies identified by inspectors had the potential to cause actual harm to residents.
