Facility Inspections
Neither this facility's overall score nor its category grades gave us many positives to write about. This place's best category was inspections, but even there we gave it a D. This is still a very poor score. One of the most significant factors in assessing inspection reports is deficiencies. We suggest you pay close attention to the severity of the deficiencies. This nursing home had a category G through L deficiency, which are among the more serious deficiencies. These categories indicate that the deficiencies identified by inspectors had the potential to cause actual harm to residents. Another red flag is that CMS cited this nursing home for possible abuse or neglect. Hopefully, its performance improves in the future, but until then we simply can't recommend this facility.
Short-term Care Quality
Tragically, this facility received an F for its short-term care score. Our short-term care scores are presumably most meaningful for folks needing rehabilitation from their nursing home. Rehabilitation typically mandates higher levels of skilled nursing. Skilled nursing means not just nursing, but also physical and speech therapy, as well as other variations of therapy. Not surprisingly, we learned that this nursing home offers substantially less physical therapist and registered nurse hours per resident than the average nursing home. The final datapoint we considered in this area is the percentage of patients who were able to leave the nursing home and return to the community. We discovered that just 0 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar statistics sunk this facility's short-term care score.

Long-term Care Quality
Our third category we assessed was long-term care, where this facility received an F. In a long-term care setting, the primary goal is to maintain residents' quality of life and keep them safe. In addition to considering the quantity of care provided by aids and other staff, we analyzed the percentage of residents vaccinated against pneumonia. Fortunately, this facility vaccinated 99.70105 percent of its patients, which is a very solid percentage. To our surprise, this nursing home also fared well at keeping its patients out of the hospital. Indeed, it had just 0.01 hospitalizations per 1,000 long-term resident days. Sadly, a few of its other scores in the category were not as favorable as these.
Nurse Quality
The next area we assessed was nursing, in which this facility received a bottom of the barrel score in this area also. We gave it an abysmal F for this category, which is definitely somewhat alarming. Nursing scores are tied to quantity of nursing care available. This facility offered just 0.2 hours of nursing care per patient on a daily basis. This is well below average. Lastly, we also assessed some nursing quality-based metrics in calculating our nursing ratings. This facility fared well when it comes to minimizing its residents' major falls and pressure ulcers.
