Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Our inspection scores weigh several factors included in a facility's inspection report. One key criteria we look at is the quantity and severity of deficiencies. This particular nursing home received 7 deficiencies on its inspection report. On top of having severe deficiencies, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.

Sadly, this nursing home's next best category is long-term care. Nevertheless, with a D in this category, it still fared relatively poorly. Facilities that do not fare well in long-term care typically are not as well-staffed and are lagging in some of the areas of routine medical care we looked at. After we finished looking at the amount of care provided by nurses, we turned to the nursing home's vaccination record. Frankly, we were a bit concerned this facility vaccinated just 85.71429 percent of its patients against pneumonia. Finally, we looked at the facility's number of hospitalizations. Here we found that this nursing home had 2.25 hospitalizations per one thousand long-term resident days. Unfortunately, this figure is also a cause for concern.

In addition, we gave this facility an F in the area of nursing care. The nursing grade includes quite a few components, however, the primary one is the level of nurse hours spent with patients. This facility provided a meager 1.7 hours of nursing care per patient on a daily basis. This is well below average. Finally, we also looked at a few quality-based metrics in determining our nursing grades. This place was above average in two of the areas we focus on, with solid scores for avoiding pressure ulcers and major falls.

Short-term Care Quality

The final category we assessed is short-term care. This facility was given a bottom of the barrel grade of F in that category. In computing our short-term care ratings, we analyze a nursing home's levels of skilled nursing services, which includes registered nurses, physical therapists speech therapists and other licensed professionals. This area is typically a solid measure of a nursing home's ability to rehabilitate patients. Considering its score in this category, we were not stunned to discover that this place is well below average in terms of its number of physical therapy and registered nurse hours offered to its residents based on the metrics we assessed. Honestly, this was about what we expected here. Finally, we considered the number of residents who who were able to eventually return home from this nursing home. This facility didn't fare well here either. In fact, we found that just 16.9 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.
