We also awarded this nursing home a middle of the pack score in the area of long-term care, where we awarded it a grade of B-. Long-term care grades are generally used to score a facility's performance as a traditional nursing home, as opposed to focusing more on the skilled nursing services offered by a rehabilitation facility. After looking at the quantity of care provided by nurses and other staff, we then looked at the facility's vaccination records. Unfortunately, this facility vaccinated a relatively low 60.946743 percent of its patients for pneumonia. Nevertheless, this facility was able to keep its patients out of the hospital. Indeed, it had just 1.44 hospitalizations per 1,000 long-term resident days.

We also wanted to point out that this nursing home received an F for its nursing grade. The nursing score is based on several subcategories, however, the primary one is the level of nurse hours per patient per week. This facility averaged only 3.1 hours of nursing care per resident daily. On top of its subpar nursing hours, this nursing home also earned poor scores in some of the quality-based measures we looked at in determining our nursing scores. We found that a higher percentage of this place's patients suffered falls which led to serious injury. We would argue that many falls could likely be prevented with better nursing care. This metric contributed to this place receiving a terrible nursing grade.

Turning to our fourth category, this facility really didn't perform well here either. With a rock bottom grade of F in short-term care, this is just about as bad as it gets. In forming these short-term care ratings, we look at the nursing home's skilled nursing services, such as those performed by registered nurses, speech therapists, physical therapists and other types of skilled professionals. The objective is to devise a tool for sizing up the rehabilitation services of different nursing homes. With its grade in this category, we weren't stunned to find that this nursing home was well below average in terms of its quantity of physical therapy and registered nurse hours offered to its residents based on the measures we assessed. Lastly, we assessed the number of patients who were able to return to the community from this facility. This was not a strength for this nursing home. We found that just 44.4 percent of this facility's patients returned home. At most nursing homes, about half of their residents are able to return home so this is well below average.
