Facility Inspections
In addition to being an elite facility overall, this facility also performed well in our inspections category, where it earned a grade of A+. Few places performed this well in this area. Inspection scores account for several factors included in a facility's inspection reports. One of the most important criteria we consider is the number and severity of deficiencies. Facilities with better grades in this category usually have few severe deficiencies. Fortunately, although this place had some minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G through L. This means that CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Short-term Care Quality
This nursing home also was superb in the area of short-term care, where we gave it a score of A. Very few facilities fared better in this area. Our short-term care grade is often used to judge a facility's rehabilitation services. To provide highly scored rehabilitation services, nursing homes generally must offer better levels of highly skilled nursing services. Skilled nursing includes registered nurses, physical therapists, occupational therapists and other highly trained professionals. Fortunately, it appear that this nursing home employs registered nurses. Not every facility employs these types of nurses. On the other hand, based on the data they provided, they do not appear to employ physical therapists. Lastly, we considered the number of patients who were able to return to the community from this facility. We found that 5.1 percent of this nursing home's residents returned home.

Long-term Care Quality
We also wanted to alert you to the fact that this nursing home was given a strong grade of A in our long-term care area. Nursing homes that excel in long-term care tend to be well-staffed and offer extensive hands on care to patients. After assessing the quantity of care provided by nurses and other staff, we next considered the facility's vaccination record. This facility vaccinated 100 percent of its patients against pneumonia. Vaccines are critical to keeping patients healthy. Clearly, this facility is doing something right in this area as it also excelled at keeping its patients out of the hospital. It had only 0.02 hospitalizations per 1,000 long-term resident days, which is an impressively low number.
Nurse Quality
In the last category we assessed, this nursing home received an impressive grade grade of A- in our nursing category. With this grade, the facility rounded out a straight A profile. Our nursing grade is based on the facility's level of nurse staffing. We look at both the levels of training of those nurses as well as the number of hours spent with patients. This nursing home provides 0.5 hours of nursing care per resident per day. Finally, we also factored a few quality measures into our nursing grades. This facility was relatively weak in two of the major areas we focus on, with below average scores for minimizing its residents' pressure ulcers and major falls. These areas are generally reliable indicators of the quality of nursing care provided.
