This facility received a quality overall grade as described above, but we especially wanted to emphasize its nearly flawless government inspections in recent years. We gave it an A- in this category. Arguably the most significant factor we look at in determining our inspection scores is deficiencies. Deficiencies are found on a nursing home's recent inspection reports. Places with better grades in this area most likely avoided the most severe deficiencies involving patient abuse or death. While this facility had some deficiencies on its inspection report, none were severe based on CMS' scale. The fact that the deficiencies were relatively minor made us feel better about this inspection report.
Nurse Quality
This facility is also strong in the area of nursing, where it received a grade of B+. It outperformed most nursing homes in this category. Our nursing grade is primarily associated with a facility's level of nurse staffing. This nursing home provides 0.3 hours of nursing care per resident per day. Finally, our nursing grades also factor in quality-based assessments, such as avoiding major falls. This place performed well in this area. Avoiding major falls is typically a good indicator that a nursing home has reliable quality controls in place. Major falls can generally be prevented if more nurses aids and better safety protocols are in place.

Long-term Care Quality
In addition, this facility also received favorable long-term care grades this year. Indeed, we awarded them one of our stronger scores in that category, with a score of B. In a long-term care setting, the primary objective is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. After considering the amount of care provided by nurses, we then analyzed the nursing home's vaccination records. This nursing home vaccinated 69.5 percent of its patients against pneumonia. Although its vaccination rate was a bit lower than we hoped for, at least this facility was able to limit hospitalizations. It had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low number.

Our final area is short-term care, where we awarded this facility a B- in that area. With our short-term care category, we attempt to craft a fair barometer for rehabilitation services. In this process, we analyze a nursing home's scope of highly-skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. Fortunately, it looks like this nursing home has registered nurses on staff. Not every nursing home employs these types of nurses. On the other hand, according to the information this nursing home provided, they do not appear to employ physical therapists. The last measure we looked at in this category is the percentage of residents that who were able to eventually return home from the nursing home. We found that just 4.9 percent of this facility's patients returned home.
