Facility Inspections

Grade: B-plus

Although we didn't give this place a good overall grade, we actually gave it a strong grade in the category of inspections, where it earned a B+. These inspection grades weigh several factors, including deficiencies and federal fines. You can find more information about each of these factors by reviewing copies of nursing homes' inspection reports. This facility was assessed 5 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered severe. This indicates that CMS did not deem any of the deficiencies to pose an imminent threat to resident health or safety. A few minor deficiencies aren't necessarily the end of the world.

Long-term Care Quality

Grade: B-minus

One of this nursing home's more favorable category scores came in the category of long-term care. This ended up being its second most favorable category. In that area, we gave this facility a grade of B-. If you are looking for services other than short-term rehabilitation, you should take a close look at long-term care scores. After we finished looking at the volume of care provided by nurses, we then analyzed the nursing home's vaccination records. Our short-term care grade is typically employed to judge a facility's performance with rehabilitation In order to provide good rehabilitation services, facilities generally need to offer higher levels of highly skilled nursing. Skilled nursing includes registered nurses, physical therapists, occupational therapists and other highly skilled individuals. Given its score in this category, we weren't surprised to learn that this place was well below average in terms of the quantity of registered nurse and physical therapy hours offered to its residents based on the measures we looked at. Frankly, this was what we expected here. The final metric we considered in this area is the number of patients who were able to leave the facility and return to the community. We discovered that just 0 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics sunk this nursing home's short-term care grade.

This nursing home's worst category was nursing, which is the final category we analyzed. For this area, we gave this nursing home a grade of D.
