Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. We take several aspects of a facility's inspection report into consideration in calculating these inspection ratings. One critical factor is health deficiencies. You should keep in mind that the severity of the deficiencies is usually more meaningful than the number of deficiencies, as some are quite minor. This place received 2 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered severe. This tells you that CMS didn't consider any of these deficiencies to cause an immediate risk to patient health or safety. A few minor deficiencies are not necessarily the end of the world.
Short-term Care Quality
Surprisingly, we awarded this nursing home a C for its short-term care rating, which is not a bad grade. Our short-term care scores are presumably most important for people requiring rehabilitation from their nursing home. Rehabilitation typically requires additional skilled nursing. Skilled nursing means not merely nursing, but also physical and respiratory therapy, as well as other forms of therapy. Unfortunately, this nursing home was below average in terms of its quantity of registered nurse and physical therapy hours provided to its residents based on the measures we assessed. Lastly, we considered the percentage of patients who returned home from this nursing home. We found that 0 percent of this facility's patients were able to return home. This figure was well below average.

Nurse Quality
We were stunned to find that this facility also excelled in the area of nursing. We gave it an D for that category, which is one of our highest grades. In determining a nursing home's nursing rating, we consider the number of hours nurses are caring for patients as well as the level of licensure of the nurses. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Lastly, we also looked at some nursing quality measures in determining our nursing scores. This nursing home fared well when it comes to minimizing its residents' pressure ulcers and major falls.
Long-term Care Quality
The last category we scored is long-term care. This nursing home received a rock bottom grade of F in this area. Facilities that receive this kind of grade in long-term care likely don't provide the type of consistent around the clock care that some other facilities provide. After we finished assessing the volume of care provided by nurses, we turned to the nursing home's vaccination record. Candidly, we were a bit worried that this nursing home vaccinated a relatively low 90.71429 percent of its patients for pneumonia. To our surprise, this place was actually decent at keeping its patients out of the hospital. In fact, it had only 0.02 hospitalizations per one thousand long-term resident days. This is its best score in this category.
