While this nursing home's overall grade was not bad, it really excelled in the area of inspections. In fact, we awarded it a grade of A for that category, which is one of our highest scores. Inspection grades take several factors into consideration, including deficiencies and federal fines. You can learn more about each of these items by obtaining copies of nursing homes' inspection reports. While this nursing home had some minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are ones labeled as categories G through L. This tells you that CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is always a good sign.

Another strength of this nursing home is its top-shelf grade in long-term care. We awarded it one of our most impressive scores in that category, with a grade of A-. When facilities receive a grade in this range in this category it generally means it has plenty of staff and is an overall good place to reside on a permanent basis. In addition to assessing the quantity of care provided by nurses and other staff, we also looked at the number of residents vaccinated against pneumonia. This facility gave the vaccine to 100 percent of its patients. Vaccines are critical to keeping patients out of the hospital. The last statistic we looked at is the nursing home's hospitalization rate. We found that this place had 1.99 hospitalizations per 1,000 long-term resident days, which is middle of the pack in this area.

Another strength of this facility is its favorable grade in nursing. We gave them one of our better scores in that area, with a score of D. We scrutinized the qualifications of nurses working for the facility, as well as the quantity of hours those nurses were with residents, in determining our grade in this area. This nursing home provided a meager 3.1 hours of nursing care per resident daily. This is a very low total compared to most nursing homes. Finally, we also looked at a few quality-based metrics in determining our nursing ratings. This nursing home was above average in two of the areas we focus on, with solid marks for minimizing its residents' pressure ulcers and major falls. These datapoints are usually good measures of the quality of nursing care provided.

Historical

This facility's least impressive category was short-term care, which is the last area we assessed. Unfortunately, we gave it a lowly F for this category. Even with several solid grades in other categories, this grade is nevertheless concerning so we felt obligated to point it out. In determining these short-term care ratings, we assess the facility's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. The objective is to devise a barometer for sizing up the rehabilitation services of nursing homes. As you might expect, we discovered that this facility provides substantially less registered nurse and physical therapist hours per patient than the average facility. Lastly, we looked at the percentage of patients who were able to return to the community from this facility. This wasn't a strength for this nursing home. We found that just 28 percent of this nursing home's patients returned home. At most nursing homes, around half of their patients are able to return home so this is well below average.
