Facility Inspections
While we were not very complementary of this nursing home's overall grade, it actually received a very impressive inspection report this year. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our best scores in that category with an A. We take several aspects of a facility's inspection report into consideration in determining these inspection grades. One of those factors is health deficiencies. It should be noted that the severity of deficiencies is usually more meaningful than the number of deficiencies, as some of these can be quite minor. This place was hit with 5 deficiencies on its inspection report, but none were considered to be severe deficiencies. This indicates that the government inspectors didn't consider any of these deficiencies an imminent threat to patient health or safety. A couple minor deficiencies aren't necessarily the end of the world.
Long-term Care Quality
Moving on to the category of long-term care, this facility was awarded a grade of just D in this area. As it turns out, this ended up being one of its better features. When facilities receive this type of score in long-term care it is often a bad sign for patient care and it may indicate that the nursing home is not as well-staffed. Once we assessed the volume of care provided by aids and other staff, we then looked at the nursing home's vaccination record. Candidly, we were a bit worried that this nursing home vaccinated 27.891155 percent of its patients against pneumonia, which happens to be quite a few points lower than average. To our surprise, this nursing home actually fared well at keeping its residents out of the hospital. In fact, it had only 0.01 hospitalizations per one thousand long-term resident days. This is its best score in this category.

Nurse Quality
This facility's next best category was nursing, but even there it received a grade of just D. This is still a well below average grade. There are a variety of factors included in this grade. Most of the factors are tied to staffing levels. With only 0.3 hours of nursing care per resident per day, this nursing home's nurse staffing levels were significantly below the national average. Finally, we also assessed some nursing quality-based metrics in computing our nursing scores. This facility performed well in the area of minimizing its residents' major falls and pressure ulcers.
Short-term Care Quality
The next area we analyzed is short-term care, ended up being this nursing home's worst category. We gave this facility a grade of D in that area. In the category of short-term care, we attempt to assess indicators of a facility's rehabilitation services. We assess a facility's skilled nursing services, such as the ones performed by registered nurses and physical, occupational and other therapists. With a grade this poor for this area, we weren't shocked to learn this facility was well below average in the two key staffing metrics we looked at. The nursing home supplied far fewer registered nurse and physical therapist hours per patient than most facilities. Finally, we considered the number of patients who were able to return to the community from this nursing home. We found that just 0 percent of this facility's residents returned home.
