Short-term Care Quality

Grade: D

Unfortunately, we did not have any favorable things to say about this nursing home's overall score or its category grades. Its best category was the area of short-term care. However, with a D in this area, it still performed poorly. Our short-term care grade is typically used to judge a nursing home's performance with rehabilitation. In order to provide highly scored rehabilitation services, nursing homes generally must feature higher levels of skilled nursing services. These types of services include registered nurses, physical therapists, occupational therapists and other highly skilled individuals. Considering its weak score in this category, we weren't stunned to learn this nursing home offered significantly fewer physical therapist and registered nurse hours per patient than most other facilities. The final item we looked at in this area is the percentage of residents who were able to leave the nursing home and return to the community. We discovered that just 0 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar metrics doomed this facility's short-term care grade.

Nurse Quality

Grade: D

One of the other reasons we rated this nursing home so highly is that it received a very impressive nursing grade. This turned out to be its second best category grade. In that category, we gave this facility an D. When calculating a facility's nursing score, we assess the number of hours nurses are caring for patients as well as the levels of training of those nurses. This facility averaged only 0.3 hours of nursing care per resident each day. Lastly, we also looked at some nursing quality-based metrics in calculating our nursing grades. This nursing home performed well when it comes to avoiding major falls and pressure ulcers. Many consider these metrics to be good measures of the quality of nursing care.

Facility Inspections

Grade: F

Among this facility's worst category grades was an F for its inspection grade. Typically, this means we found some red flags on the facility's government inspections. When a facility has this bad of an inspection score, you should expect some severe deficiencies on its inspection report. Severe deficiencies indicate a risk to the well being of residents. This particular nursing home was assessed 6 deficiencies by CMS. The only positive thing we found is that none of its deficiencies were in the categories that indicate they posed a threat to resident safety or health. Finally, CMS cited this nursing home for possible abuse or neglect. Hopefully, its performance improves in the future, but until then we simply can't recommend this facility.

Long-term Care Quality

Grade: F

The final category we scored was long-term care. This nursing home received a rock bottom grade of F in this area. For prospective patients in need of a permanent residence as opposed to skilled nursing, long-term care is a very important category. Once we assessed the quantity of care provided by aids and other staff, we next considered the nursing home's vaccination records. This nursing home vaccinated 94.210526 percent of its residents for pneumonia, which is higher than most nursing homes. To our surprise, this nursing home also fared well at keeping its patients out of the hospital. In fact, it had just 0.02 hospitalizations per one thousand long-term resident days. Sadly, a few of its other scores in this category were not as favorable as these.
