Long-term Care Quality

Grade: B-minus

Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our long-term care rating. If you are looking for services other than rehabilitation, you should take a close look at each nursing home's long-term care scores. One of the factors we considered on top of nursing hours is vaccines. Thankfully, this nursing home administered the pneumonia vaccination to 99.76019 percent of its residents. This nursing home also excelled at keeping its residents out of the hospital. It had only 0.02 hospitalizations per one thousand long-term resident days, which is an impressively low number.

Facility Inspections

Grade: C

As it turns out, its government inspection reports were not the source of this nursing home's rock bottom overall grade. It earned a C in this category. Our inspection scores are tied to pieces of information located in the a nursing home's recent inspection reports. Deficiencies are a key item to look for on these inspections. Severe deficiencies indicate a risk to patient safety. This place had 3 deficiencies on its inspection report, but none were in categories G through L. This indicates that the government inspectors didn't consider any of the deficiencies to be an imminent threat to resident safety or health. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.

Nurse Quality

Grade: D

Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. When computing our nursing scores, we look at both staffing levels and the skill levels of those nurses. This facility averaged only 0.4 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Lastly, we also looked at some nursing quality-based metrics in determining our nursing grades. This place performed well in the area of minimizing its residents' major falls and pressure ulcers. Many consider these metrics to be good indicators of the quality of nursing care.

Short-term Care Quality

Grade: F

The next category we rated was short-term care, in which this facility was given an abysmal grade here also. Unfortunately, we gave it a lowly F for this category, which is definitely a major concern. In the area of short-term care, we endeavor to assess indicators of a facility's rehabilitation services. We analyze the nursing home's skilled nursing services, including those performed by registered nurses and physical and occupational therapists. With its grade in this category, we were not stunned to learn that this facility was well below average in terms of its number of physical therapy and registered nurse hours provided to its patients based on the measures we assessed. The last item we looked at in this category is the number of patients who were able to leave the nursing home and return to the community. We discovered that just 4.2 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar metrics doomed this facility's short-term care score.
