Facility Inspections
While we were not very complementary of this facility's overall score, it actually received a very impressive inspection report this year. In fact, it received an excellent inspection report this year. As a result, it received one of our best grades in that category with an A. Perhaps the most important factor we consider in determining our inspection grades is deficiencies. These deficiencies can be found on a facility's inspection reports. It is typically in your best interest to avoid nursing homes that have a long list of deficiencies. This particular nursing home received 8 deficiencies on its inspection report. Overall, this is still a solid inspection report, but we'd always recommend looking into any deficiencies you find troubling.
Long-term Care Quality
This facility's second highest area is long-term care. In that category, we awarded this facility a grade of C. For prospective residents in need of a permanent residence as opposed to rehabilitation, long-term care is a very important category. One of the criteria we considered on top of nursing hours is vaccines. Thankfully, this facility provided the pneumonia vaccination to 100 percent of its patients. This place keeps its patients out of the hospital. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low number.

This nursing home's second lowest category is short-term care, where it earned a grade of just D in that area. This is a well below average score. Short-term care ratings are critical for individuals seeking rehabilitation. Rehabilitation generally requires additional skilled nursing. Skilled nursing includes a vast range of services, ranging from registered nurses to physical and respiratory therapists, as well as other forms of therapy. With a score in this range for this category, we weren't stunned to find that this facility was below average in the two key staffing metrics we assess. The facility offered substantially fewer registered nurse and physical therapist hours per resident than most other nursing homes. Finally, we assessed the percentage of residents who were able to return to the community from this facility. This was not a strength for this nursing home. We found that just 4.4 percent of this facility's residents returned home. At most nursing homes, around half of their residents are able to return home so this is well below average.
Nurse Quality
The next area we assessed was nursing, where this nursing home received a bottom of the barrel grade here as well. We gave it an abysmal F in this area, which is definitely very concerning. When calculating a nursing home's nursing score, we consider the quantity of hours nurses commit to patients as well as the levels of training of the nurses. This facility offered just 0.3 hours of nursing care per patient per day. To go along with its low total nursing hours per resident, this nursing home had bad scores in several of the quality-based metrics we looked at in calculating our nursing scores. We looked at the percent of patients experiencing falls which led to major injury. This nursing home was at more than 150 percent of the national average in this statistic. This is quite concerning in light of the fact that so many falls could likely be avoided with better nursing care. This ended up sealing the nursing home's fate of receiving a poor nursing rating.
