Facility Inspections
B-
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. These inspection grades take several factors into consideration, including deficiencies and substantiated complaints. You can find more information about each of these items by obtaining copies of nursing homes' inspection reports. While this facility had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are those labeled as categories G through L. This means that the government inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Long-term Care Quality
B-
This facility also didn't grade out too badly in the area of long-term care. Indeed, we gave it a grade of B- for that category, which is among its higher scores. In a long-term care environment, the primary objective is to maintain residents' quality of life and keep them safe. On top of assessing the quantity of care provided by nurses aids and other staff, we looked at the percentage of residents vaccinated against pneumonia. This nursing home administered the vaccine to 90.7563 percent of its patients. This is several points less than what we were hoping for. Unfortunately, this nursing home's hospitalization rate was also below average. We found that this facility had 2.13 hospitalizations per 1,000 long-term resident days. While this figure is quite a bit higher than the national average, this number may be skewed for some nursing homes based on the medical complexity of residents

Nurse Quality
F
Unfortunately, we gave this facility an F for its nursing grade. The nursing grade analyzes quite a few subcategories, however, the primary one is the number of nurse hours spent with patients. This nursing home offered a meager 2.2 hours of nursing care per resident daily. This is an alarmingly low total. On top of its weak nursing hours, this nursing home earned suboptimal scores in many of the quality-based metrics we considered in calculating our nursing ratings. We found that more of this facility's patients suffered falls leading to major injury. We would argue that many falls could be avoided with better nursing care. This statistic contributed to this place receiving a terrible nursing grade.
Short-term Care Quality
F
The final area we graded is short-term care. Sadly, it received an abysmal F for this category, which is a rock bottom grade. Our short-term care scores are based on the facility's quantity of highly-skilled nursing services. This includes a wide spectrum of services, spanning from registered nurses to physical and respiratory therapists, as well as other variations of therapy. As you might expect, we discovered that this facility offers substantially less physical therapist and registered nurse hours per patient than the typical facility. The final datapoint we looked at in this area is the number of patients who were able to leave the facility and return to the community. We discovered that just 12.3 percent of this facility's residents were able to return home, which is well below average. The combination of these poor statistics was damning for this nursing home's short-term care score.
