Facility Inspections
This place's grades were horrible across the board. In particular, we wanted to point out its inspection grade where we gave it an F. This isn't a good sign for its inspection reports. For places with poor inspection grades, you should really focus on any severe deficiencies found on their inspection reports. This nursing home was hit with 5 deficiencies on its inspection report. The only positive thing we found is that none of the deficiencies were in the categories that suggest that they created a threat to patient safety or health. Lastly, this facility was flagged by CMS for possible abuse or neglect. We prefer to avoid facilities with this designation. Please do your best to find an alternative nursing home.
Long-term Care Quality
We also wanted to point out this facility's low-end long-term care score where it received a lowly F. When facilities receive this type of score in this category it is a bad sign for patient care and it may indicate that the facility is not as well-staffed. One of the factors we considered in addition to nursing hours is vaccinations. Frankly, we were a bit alarmed this facility administered the pneumonia vaccine to just 79.71831 percent of its patients. Surprisingly, this place was actually able to limit hospitalizations. With just 0.02 hospitalizations per 1,000 long-term resident days, this nursing home had fewer hospitalizations than most nursing homes. This was its best score in this category.

Nurse Quality
This facility ended up receiving rock bottom score in our nursing category. It received an F in this category. Our nursing grade weighs many components, but the most important one is the number of nurse hours per patient per week. This place averaged only 0.3 hours of nursing care per resident on a daily basis. Lastly, we also assessed some nursing quality measures in calculating our nursing grades. This place fared well when it comes to minimizing its residents' pressure ulcers and major falls.
Short-term Care Quality
Turning to our next category, this nursing home did not fare well in this category either. Due to an abysmal grade of F in short-term care, this is just about as bad as it gets. In the category of short-term care, we try to qualify indicators of a facility's rehabilitation services. We assess the nursing home's skilled nursing services, including those performed by registered nurses and physical and occupational therapists. With its abysmal grade in this area, we weren't shocked to learn this nursing home offered significantly fewer registered nurse and physical therapist hours per patient than most other facilities. Finally, we looked at the percentage of residents that were able to return home from this facility. This nursing home didn't fare well here either. In fact, we found that just 4.9 percent of this facility's patients were able to return home. This figure was well off the national average.
