Facility Inspections

Grade: C

Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection ratings weigh several factors, including deficiencies, substantiated complaints and federal fines. You can find more information about each of these factors by reviewing copies of nursing homes' inspection reports. This facility was assessed 6 deficiencies on its inspection report, but we were relieved to find that none were category G through L deficiencies. This tells you that CMS didn't consider any of the deficiencies to pose an imminent risk to patient health or safety. Unfortunately, we also need to draw your attention to the fact that this facility received some government fines in recent years.

Long-term Care Quality

Grade: D

Unfortunately, this facility's second most favorable category is long-term care. Nevertheless, with a grade of D in this category, it still fared relatively poorly. When nursing homes receive this kind of grade in this category it is often a bad sign for patient care and it may indicate that the place is not as well-staffed. One of the statistics we considered on top of nursing hours was vaccines. This facility administered the pneumonia vaccine to 93.83886 percent of its residents. Surprisingly, this facility actually fared well at keeping its residents out of the hospital. In fact, it had just 0.01 hospitalizations per 1,000 long-term resident days. This was its best feature in this category.

Nurse Quality

Grade: D

We gave this facility a grade of just D for our nursing rating. Nursing scores are heavily correlated with levels of nurse staffing. This nursing home provided a meager 0.4 hours of nursing care per patient each day. This is not a very impressive figure. Lastly, we also assessed some nursing quality-based metrics in determining our nursing ratings. This nursing home performed well when it comes to avoiding major falls and pressure ulcers. We consider these areas to be measures of the quality of nursing care.

Short-term Care Quality

Grade: F

The last category we scored was short-term care. Sadly, it received a lowly F for this area, which is a rock bottom score. In computing our short-term care scores, we look at the facility's levels of skilled nursing, which includes registered nurses, physical therapists speech therapists and other licensed professionals. This area is typically a meaningful assessment of a facility's rehabilitation. As you might expect, we discovered that this place provides far less registered nurse and physical therapist hours per resident than the average facility. The final datapoint we assessed in this area is the number of residents who were able to leave the nursing home and return to the community. We discovered that just 4.8 percent of this facility's residents were able to return home, which is well below average. The combination of these poor metrics doomed this facility's short-term care score.
