Facility Inspections
Although we gave this place a terrible overall score, we gave it a grade of B+ for our inspections category. This is due to the nursing home receiving a decent government inspection report this year. Our inspection grades are tied to many datapoints located in the a nursing home's recent inspection reports. Deficiencies are an important item to look for on these reports. You should especially avoid facilities that have severe deficiencies linked to endangerment of patients. Fortunately, although this nursing home had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are those found in categories G, H, I, J, K and L. This tells you that CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A few minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
As it turns out, this nursing home only received a D for its long-term care rating, which is actually one of its more favorable category grades. When facilities receive a score in this range in this category it generally means the facility did not perform well in our quality measures relating to patient care. After assessing the volume of care provided by aids and other staff, we turned to the nursing home's vaccination data. Frankly, we were a bit alarmed this nursing home vaccinated 59.0625 percent of its residents for pneumonia, which happens to be several points below what we were hoping for. Surprisingly, this facility was actually decent at keeping its patients out of the hospital. It had only 0 hospitalizations per 1,000 long-term resident days. This is its best score in this category.

Short-term Care Quality
The last category we assessed is short-term care, where this nursing home received a bottom of the barrel score in this category also. We gave it an F in this area, which is definitely a major concern. In calculating our short-term care scores, we scrutinize the nursing home's levels of skilled nursing, which includes registered nurses, physical therapists speech therapists and other highly skilled individuals. This grade is frequently a meaningful measure of a nursing home's ability to rehabilitate patients. Based on its score in this category, we weren't surprised to find that this facility was well below average in terms of its number of registered nurse and physical therapy hours offered to its patients based on the measures we looked at. Honestly, this was about what we expected to find. Lastly, we looked at the percentage of residents who returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 4.2 percent of this facility's patients were able to return home. This figure was quite a bit off the national average.
