This facility ended up receiving rock bottom scores in our assessment. Perhaps most alarmingly, it received an F for its long-term care grade. If you are seeking anything other than rehabilitation, you should take a close look at each facility's long-term care scores. After considering the amount of care provided by nurses and other staff, we then looked at the facility's vaccination record. We were optimistic to discover that this facility vaccinated 98.56322 percent of its residents against pneumonia. To our surprise, this nursing home was able to limit hospitalizations. With just 1.39 hospitalizations per 1,000 long-term resident days, this facility had fewer hospitalizations than the average nursing home. Unfortunately, some of its other scores in this category weren't as favorable as these.

We also want to note that this nursing home was given an F for its short-term care score. Our short-term care scores are believed to be most meaningful for individuals needing rehabilitation from their nursing home. Rehabilitation usually requires more skilled nursing services. This means not merely nursing, but also physical and speech therapy, as well as other forms of therapy. With a grade this poor in this category, we were not shocked to learn this nursing home was below average in two of the key staffing metrics we focus on. The nursing home supplied far fewer physical therapist and registered nurse hours per resident than most other nursing homes. The final datapoint we considered in this category is the number of patients who were able to leave the facility and return to the community. We discovered that just 46.2 percent of this facility's patients were able to return home.

We wanted to point out that this nursing home received an F for its inspection rating. An F in this category is particularly troublesome because it usually points to red flags on the nursing home's government inspection reports. With places that received this bad of an inspection score, we recommend scrutinizing the severe deficiencies on its inspection report. This facility was assessed one category G through L deficiency, which are among the more severe deficiencies. These categories indicate that the deficiencies found by CMS had potential to cause actual harm to patients. Lastly, we also want to point out that this place was assessed large government fines in excess of $50,000. This generally indicates that CMS inspectors became fed up after multiple years of failed government inspections.

The next category we assessed is nursing, in which this facility was given a poor score in this category as well. It received an abysmal F for this category, which is obviously a major concern. The nursing score consists of quite a few data points, but the most important consideration is the level of nurse hours per patient per week. This place provided only 2.2 hours of nursing care per patient each day. Lastly, we also looked at some quality-based metrics in calculating our nursing ratings. This facility was above average in two of the major statistics we considered, with solid scores for minimizing its residents' pressure ulcers and major falls.
