Facility Inspections
While this facility's overall grade was as bad as it gets, it actually really excelled in the area of inspections. In fact, we awarded it an A- for that category, which is one of our highest scores. We weigh several aspects of a nursing home's inspection report in determining these inspection grades. One of those factors is health deficiencies. You should keep in mind that the severity of deficiencies is usually more meaningful than the quantity of deficiencies, as some of these are quite minor. While this nursing home had a few minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This tells you that the inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A few minor deficiencies aren't necessarily the end of the world.
Short-term Care Quality
Moving on to the area of short-term care, this facility was given a grade of just D in this area. As it turns out, this proved to be one of its better grades. Our short-term care grade is commonly employed to gauge a nursing home's rehabilitation services. To have quality rehabilitation services, facilities generally need to offer better levels of highly skilled nursing. Skilled nursing includes registered nurses, physical therapists, occupational therapists and other highly trained professionals. Given its grade in this area, we were not surprised to discover that this nursing home was well below average in terms of its number of registered nurse and physical therapy hours offered to its patients based on the data we assessed. Finally, we assessed the percentage of residents that ultimately returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 5.7 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.

Long-term Care Quality
In addition, we gave this facility a rock-bottom F in the area of long-term care. For prospective residents in need of a permanent residence as opposed to skilled nursing, long-term care grades are an important measure. After considering the amount of care provided by aids and other staff, we next considered the nursing home's vaccination records. We were optimistic to learn that this facility vaccinated 96.55173 percent of its patients against pneumonia. Surprisingly, this nursing home was able to limit hospitalizations. With just 0 hospitalizations per 1,000 long-term resident days, this facility has fewer hospitalizations than many nursing homes. Sadly, some of its other scores in this category were not as favorable as these.
Nurse Quality
The next area we assessed was nursing. This nursing home received a rock bottom grade of F in this category. Nursing grades are primarily associated with a nursing home's level of nurse staffing. This place offered just 0.2 hours of nursing care per patient per day. This is not a very impressive figure. Lastly, we also looked at some nursing quality-based metrics in computing our nursing ratings. This nursing home performed well in terms of avoiding pressure ulcers and major falls.
