Facility Inspections
The primary reason this turned out to be a decent nursing home is that it earned an excellent inspection grade. In fact, its inspection score was far superior to its overall score. In the inspections category, we gave this facility an A+. Inspection scores take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these items by obtaining copies of nursing homes' inspection reports. Although this facility had a few deficiencies on its inspection report, none of them were severe based on CMS' deficiency scale. A couple minor deficiencies shouldn't stop you from considering a nursing home.
Short-term Care Quality
This facility's second most favorable area was its short-term care score, where it was awarded a grade of B-. In the area of short-term care, we strive to evaluate indicators of a facility's rehabilitation services. We look at the nursing home's skilled nursing services, such as those performed by registered nurses and physical and occupational therapists. Unfortunately, this facility was below average in terms of its quantity of registered nurse and physical therapy hours provided to its patients based on the metrics we assessed. The last item we considered in this area is the percentage of patients who were able to return home from the nursing home. We found that just 6.6 percent of this nursing home's patients returned home.

Long-term Care Quality
This nursing home received its third highest category grade in the category of long-term care. We gave this facility a C in this category, which is an acceptable grade. For patients seeking a permanent place to live rather than skilled nursing, long-term care grades are an important measure. One of the criteria we considered on top of nursing hours was vaccinations. Fortunately, this nursing home provided the pneumonia vaccination to 100 percent of its residents. Finally, this nursing home was also able to limit hospitalizations. It had only 0.02 hospitalizations per 1,000 long-term resident days, which is an impressively low number.
Nurse Quality
This facility's least impressive category is nursing, which is the final area we assessed. Unfortunately, we gave it an abysmal F for this category. In spite of a few solid grades in other categories, this grade is nevertheless of a concern so we felt obligated to make a note of it. Our nursing rating is based on the nursing home's level of nurse staffing. We weigh both the levels of skill of those nurses as well as the amount of time spent with patients. This place averaged just 0.1 hours of nursing care per resident per day. On top of ranking below average in nursing hours per resident, this facility also had poor marks in the quality-based metrics we looked at in computing our nursing grades. We looked at the percent of patients sustaining pressure ulcers and major falls. This facility was at about 150 percent of the national average in both of these statistics. This is likely a bad sign when you consider that many bed sores and falls are preventable with better nursing care. These statistics pulled down this nursing home's nursing grade significantly.

In Country Village Care, 14.35% of Patients use Anti-Psychotic Medication
This datapoint indicates the percent of long-term residents which are prescribed antipsychotic medication. Increased usage of these drugs may indicate a nursing home is using these drugs to subdue residents in scenarios where such drugs aren't medically required.
