Facility Inspections
While this nursing home's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a grade of B- for that category, which is one of our better scores. We would argue that one of the most critical factors in evaluating an inspection report is deficiencies. We especially focus on the severity of the deficiencies. This nursing home was assessed 5 deficiencies on its inspection report, but none of the deficiencies were considered to be severe deficiencies. This indicates that CMS did not deem any of the deficiencies to be an imminent risk to resident safety or health. A couple minor deficiencies aren't the end of the world.
Nurse Quality
One of the other reasons we graded this facility so highly is that it earned a very impressive nursing score. In fact, nursing proved to be its second best category grade. In that category, we gave this nursing home an C. We looked at the licensure of nurses at the facility, as well as the quantity of time those nurses were with patients, in determining our grade in this category. This nursing home provides 0.5 hours of nursing care per resident daily. This is a lower figure than we are used to seeing. Finally, despite not having the most impressive total nursing hours, this facility actually performed admirably in the metric of preventing major falls. This is generally a good indicator that a facility has sufficient quality controls in place.

Short-term Care Quality
Unfortunately, this nursing home only received a grade of D for its short-term care grade, which is not a very good score. In determining our short-term care scores, we look at a nursing home's levels of highly skilled nursing services, which includes registered nurses, physical therapists occupational therapists and other types of therapists. This rating is frequently a solid assessment of the nursing home's rehabilitation services. Not surprisingly, we learned that this nursing home provides far less registered nurse and physical therapist hours per resident than the average facility. The final measure we considered in this category is the number of patients that were able to return home from the nursing home. This facility struggled quite a bit in this area as well, with just 4.7 percent of its patients returning home. Unfortunately, this was well below average.
Long-term Care Quality
The next category we scored is long-term care, ended up being this nursing home's weakest area. We gave this nursing home a D in that area. In a long-term care setting, the facility's primary objective is to maintain residents' quality of life and keep them safe. Once we looked at the amount of care provided by nurses aids and other staff, we turned to the facility's vaccination statistics. This nursing home vaccinated 100 percent of its patients for pneumonia, which is more than the majority of nursing homes. To our surprise, this place also keeps its patients out of the hospital. Indeed, it had just 0.03 hospitalizations per 1,000 long-term resident days. Sadly, some of its other scores in the category were not as favorable as these.
