Facility Inspections
While this nursing home's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a grade of B- for that category, which is one of our better scores. We would argue that one of the most critical factors in evaluating an inspection report is deficiencies. We especially focus on the severity of the deficiencies. This nursing home was assessed 5 deficiencies on its inspection report, but none of the deficiencies were considered to be severe deficiencies. This indicates that CMS did not deem any of the deficiencies to be an imminent risk to resident safety or health. A couple minor deficiencies aren't the end of the world.
Nurse Quality
One of the other reasons we graded this facility so highly is that it earned a very impressive nursing score. In fact, nursing proved to be its second best category grade. In that category, we gave this nursing home an C. We looked at the licensure of nurses at the facility, as well as the quantity of time those nurses were with patients, in determining our grade in this category. This nursing home provides 0.5 hours of nursing care per resident daily. This is a lower figure than we are used to seeing. Finally, despite not having the most impressive total nursing hours, this facility actually performed admirably in the metric of preventing major falls. This is generally a good indicator that a facility has sufficient quality controls in place.

Short-term Care Quality
Remarkably, we gave this nursing home a grade of C for our short-term care grade, which isn't a bad grade. In determining our short-term care ratings, we assess a nursing home's levels of highly skilled nursing, which includes registered nurses, physical therapists occupational therapists and other types of therapists. This category is frequently a meaningful measure of the nursing home's rehabilitation services. We were surprised to find that this facility was below average in the two key staffing areas we focus on. The facility offered fewer physical therapist and registered nurse hours per resident than most other facilities. The final measure we considered in this category is the number of patients that were able to return home from the nursing home. We found that just 4.7 percent of this facility's residents returned home. This figure was below the national average.
Long-term Care Quality
Turning to our last category, this facility did not perform well in this category either. Due to a rock bottom grade of F in long-term care, this is about as terrible as it gets. For long-term care residents, the facility's primary objective is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. In addition to assessing the amount of care provided by nurses aids and other staff, we also looked at the number of residents vaccinated against pneumonia. Thankfully, this facility administered the vaccine to 100 percent of its patients, which is a very respectable percentage. To our surprise, this place also keeps its patients out of the hospital. In fact, it had just 0.01 hospitalizations per one thousand long-term resident days. Sadly, some of its other scores in the category weren't as favorable as these.
