Facility Inspections

Grade: B-minus

While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. We take several aspects of a facility's inspection report into consideration in computing these inspection ratings. One of those factors is deficiencies. You should keep in mind that the severity of the deficiencies is usually more important than the quantity of deficiencies, as some of these end up being quite minor. Although this facility had some deficiencies on its government inspection report, none were severe based on CMS' scale. A couple minor deficiencies are not necessarily the end of the world.

Short-term Care Quality

Grade: D

This nursing home's next most respectable category is short-term care, but even there it earned just a D in that category. This is still a well below average grade. Our short-term care scores are vital for patients looking for rehabilitation. Rehabilitation generally utilizes additional skilled nursing services. Skilled nursing includes a wide spectrum of nursing services, ranging from registered nurses to physical and speech therapists, as well as other forms of therapy. Given its grade in this category, we weren't surprised to learn that this facility was well below average in terms of the number of registered nurse and physical therapy hours provided to its residents based on the metrics we assessed. Lastly, we looked at the number of patients who were able to return to the community from this facility. We found that just 4.4 percent of this facility's residents returned home.

Nurse Quality

Grade: D

We were surprised to find that this facility also excelled in the category of nursing. We awarded it a score of D for that category, which is one of our highest scores. There are many datapoints included in this area. Many of the datapoints reflect the quantity and quality of nurse staffing. With just 0.3 hours of nursing care per resident each day, this facility's nurse staffing levels were far below the national average. Lastly, we also assessed some nursing quality-based metrics in calculating our nursing grades. This facility performed well when it comes to minimizing its residents' pressure ulcers and major falls. Many consider these metrics to be good indicators of the quality of nursing care.

Long-term Care Quality

Grade: F

The final category we assessed is long-term care, where this nursing home was given a bottom of the barrel grade here as well. We gave it a lowly F for this area, which is obviously a major concern. In a long-term care setting, the primary goal is to maintain patients' quality of life and keep them safe. Once we assessed the quantity of care provided by nurses, we then analyzed the facility's vaccination data. We were pleased to discover that this nursing home vaccinates 100 percent of its patients against pneumonia. Surprisingly, this nursing home was able to limit hospitalizations. With just 0.03 hospitalizations per one thousand long-term resident days, this facility had fewer hospitalizations than most nursing homes. Sadly, some of its other scores in the category weren't as favorable as these.
