With an address in Santa Maria, California, Country Oaks Care Center is one of four available nursing homes in the city. It looks like this is a fine facility. We awarded it an overall grade of B, which is a very solid score. In our view, this nursing home is a solid choice for quite a few people. The best part of this nursing home's impressive report card was its inspection reports. We discuss inspections in the next paragraph

Facility Inspections
To go along with its favorable overall grade, we gave this nursing home an A for the area of inspections. Our inspection scores account for several factors included in a facility's inspection report. One key criteria we weigh heavily is the quantity and severity of deficiencies. Places with higher scores in this category tend to have few of these severe deficiencies. This particular nursing home received 2 deficiencies on its inspection report. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Short-term Care Quality
Adding to this facility's list of accomplishments, it also was given a very favorable short-term care grade. We awarded them an elite grade of A- in this category. Our short-term care grades are based on the nursing home's quantity of skilled nursing services. This includes a broad scope of services, ranging from registered nurses to physical and respiratory therapists, as well as other types of therapy. Remarkably, this facility provides its residents with at least 50% more physical therapist hours per week than the average facility. Finally, we looked at the number of residents who were able to return to the community from this nursing home. We found that it outperformed the vast majority of facilities with 60 percent of its patients returning home. This is a significantly higher rate than most nursing homes.
Long-term Care Quality
This facility was awarded an impressive long-term care grade as well. We awarded it one of our higher scores in that area with a score of B+. When facilities receive this kind of score in long-term care it is a good sign for patient care and suggests that the place is well-staffed with nurses aids. Once we concluded our assessment of the amount of nursing care, we turned to the facility's vaccination data. This nursing home vaccinated 100 percent of its residents for pneumonia, which is an impressive figure. Pneumonia sadly is often a life or death ailment for nursing home residents so we strongly prefer when a nursing home doesn't leave its residents vulnerable. Finally, this place was able to limit hospitalizations. In fact, it had only 0.79 hospitalizations per one thousand long-term resident days, which is a remarkably low number.
Nurse Quality
The next area we analyzed is nursing, turned out to be this facility's least impressive category. We awarded this facility a grade of D in that category. This was one of the couple of weak links of a quality profile. In calculating a nursing home's nursing score, we weigh the amount of time nurses are seeing patients as well as the skill levels of those nurses. This nursing home provides 3.6 hours of nursing care per resident per day, which is slightly below the national average. Unfortunately, this place also did not perform as well as we had hoped in some of the quality-based metrics we looked at in this category.
