Facility Inspections
While we were not high on this nursing home's overall score, it actually received a very impressive inspection report this year. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our best scores in that category with an A+. We take several aspects of a facility's inspection report into consideration in computing our inspection scores. One of those factors is deficiencies. You should keep in mind that the severity of deficiencies is usually more meaningful than the number of deficiencies, as some deficiencies are relatively minor. This facility was hit with 5 deficiencies on its inspection report, but we were relieved to see that none were considered to be severe deficiencies. This means that the inspectors didn't deem any of the deficiencies to pose an immediate threat to patient safety or health. A few minor deficiencies aren't necessarily the end of the world.
Short-term Care Quality
Turning to the category of short-term care, this nursing home received a grade of just D in that area. Sadly, this wound up being one of its best areas. Short-term care grades are often employed to gauge a facility's rehabilitation services. In order to offer high-end rehabilitation services, nursing homes generally must feature better levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, speech therapists and other highly skilled individuals. Considering its abysmal rating in this area, we were not stunned to discover this facility provides far fewer registered nurse and physical therapist hours per patient than most other facilities. Finally, we looked at the number of residents that were able to return home from this facility. This facility didn't fare well here either. In fact, we found that just 5.5 percent of this facility's patients were able to return home.

Long-term Care Quality
We gave this facility just a D for our long-term care grade. This is not a very good score. Facilities that don't fare well in long-term care often do not provide as much nursing care and also may be struggling in a few of the areas of routine medical care we assessed. One of the datapoints we considered in addition to nurse's aid hours was vaccines. We were a bit worried that this facility provided the pneumonia vaccination to a relatively low 83.16832 percent of its residents. Surprisingly, this place actually fared well at keeping its patients out of the hospital. It had just 0.03 hospitalizations per one thousand long-term resident days. This is its best score in this category.
Nurse Quality
The last category we analyzed was nursing, where this nursing home received a very poor grade here also. Unfortunately, we gave it an F in this category, which is definitely somewhat alarming. There are a variety of subcategories within this area. Most of the factors reflect levels of nurse staffing. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also looked at some nursing quality measures in determining our nursing ratings. This facility performed well in terms of avoiding pressure ulcers and major falls.
