With an address in New Braunfels, Texas, Colonial Manor Care Center is one of four available nursing homes in the city. We gave this nursing home an F, which is just about as bad as it gets in our grading scheme. Unfortunately, the city grade for New Braunfels is not too much better. Nevertheless, you still should be able to do better Lastly, this facility had an ownership change in the last year. Hopefully, new ownership is able to elevate its quality standards.

Nurse Quality
While this nursing home's overall grade was as bad as it gets, it didn't perform as poorly in nursing. We awarded it a C for that area. Our nursing grade is based on many factors, most of which are associated with levels of nurse staffing. This facility provided just 3.2 hours of nursing care per patient per day. This is a relatively low total compared to most other nursing homes. Finally, our nursing scores also factor in quality-based measures, such as avoiding major falls. While this place didn't have as many nursing hours per resident as many of the other facilities we looked at, it was still better than average in the area of avoiding falls.
Long-term Care Quality
This nursing home wound up receiving a bottom of the barrel grade in our long-term care area. It was given a lowly F in this area. When nursing homes receive this kind of grade in this category it is typically a bad sign for patient care and it may mean that the nursing home is not as well-staffed with nurses and aids. One of the statistics we considered on top of nursing hours was vaccinations. This nursing home provided the pneumonia vaccination to 92.21312 percent of its residents. Surprisingly, this nursing home actually fared well at keeping its residents out of the hospital. It had only 0.98 hospitalizations per 1,000 long-term resident days. This was its best score in this category.
Facility Inspections
We also wanted to point out this nursing home's inspection score where it received an F. This often means we found red flags on the inspection reports. For facilities that received this bad of an inspection rating, we would scrutinize the severe deficiencies on its inspection report. This facility was hit with a category L deficiency, which is the worst possible category of deficiencies. This indicates CMS identified a widespread deficiency putting resident safety or health in immediate danger. Even a single deficiency of this severity can drag a nursing home's inspection rating down. Lastly, we also would like to point out that this nursing home was assessed large government fines recently totaling over $100,000. This usually indicates that a facility's poor performance spanned several years.

The last category we graded is short-term care, where this nursing home received an abysmal grade here as well. It received an F in this category, which is obviously a major concern. In the category of short-term care, we try to assess measures of a facility's rehabilitation. We look at a nursing home's skilled nursing services, including those performed by registered nurses and physical therapists. Based on its grade in this category, we were not surprised to learn that this nursing home is well below average in terms of the quantity of physical therapy and registered nurse hours provided to its residents based on the measures we looked at. The final item we looked at in this area is the number of patients who were able to leave the facility and return to the community. We discovered that just 28.8 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar metrics doomed this nursing home's short-term care score.
