Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection scores take several factors into consideration, including deficiencies and substantiated complaints. This place received 5 deficiencies on its inspection report, but we were relieved to see that none were considered to be severe deficiencies. This means that the government inspectors didn't deem any of the deficiencies to create an immediate threat to patient health or safety. A couple minor deficiencies aren't the end of the world.

In our next category, we gave this nursing home a rock-bottom F in the category of long-term care. For patients seeking a permanent residence rather than rehabilitation, long-term care is a very important category. On top of considering the volume of care provided by nurses and other staff, we looked at the number of residents vaccinated against pneumonia. Fortunately, this nursing home provided the vaccine to 97.34513 percent of its patients, which is a very solid total. Surprisingly, this nursing home was able to limit hospitalizations. With just 0.02 hospitalizations per 1,000 long-term resident days, this facility had less hospitalizations than the average nursing home. Sadly, a few of its other scores in this category weren't as favorable as these.

The next area we looked at was short-term care. This facility's was given a poor short-term care score where it was given an F. In computing our short-term care scores, we analyze the nursing home's levels of highly skilled nursing services, which includes registered nurses, physical therapists occupational therapists and other licensed professionals. This rating is typically a solid assessment of the nursing home's rehabilitation. Based on its grade in this area, we weren't stunned to learn that this facility was well below average in terms of its number of physical therapy and registered nurse hours provided to its residents based on the figures we assessed. The final datapoint we assessed in this category is the number of residents who were able to leave the facility and return to the community. We discovered that just 4.1 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar statistics sunk this facility's short-term care grade.

The last area we scored was nursing, where this facility received a poor grade in this category also. We gave it an F for this category, which is definitely somewhat alarming. Our nursing category assesses many subcategories, most of which are tied to nursing hours. This particular facility averaged just 0.3 hours of nursing care per resident each day. This number is significantly below the national average. On top of performing badly in terms of nursing hours, this place also had terrible scores in the quality-based metrics we looked at in computing our nursing scores. For starters, we looked at the percent of patients sustaining pressure ulcers and major falls. This place was at more than twice the national average in both of these statistics. This is concerning when you consider that many falls and bed sores are preventable with better nursing care. This sealed this nursing home's fate of receiving a poor nursing grade.
