Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection grades take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can find more information about each of these factors by reviewing copies of nursing homes' inspection reports. While this nursing home had a few deficiencies on its government inspection report, none of them were severe based on CMS' deficiency scale. A couple minor deficiencies aren't necessarily the end of the world.
Nurse Quality
One of the other reasons we rated this facility so highly is that it received a very impressive nursing grade. In that category, we awarded this nursing home an D. The nursing category assesses many factors, many of which are based on levels of nurse staffing. This place provided a meager 0.3 hours of nursing care per resident per day. This is not a very impressive figure. Finally, we also assessed some nursing quality-based metrics in determining our nursing grades. This place fared well when it comes to avoiding major falls and pressure ulcers.
Does this page answer your questions about Clearwater Nursing & Rehabilitation Center?
Short-term Care Quality
We awarded this nursing home a grade of just D for our short-term care score. This is well below average in this category. In determining our short-term care ratings, we scrutinize a nursing home's levels of highly skilled nursing, which includes registered nurses, physical therapists respiratory therapists and other highly skilled individuals. This area is considered to be a meaningful measure of the nursing home's rehabilitation. Given its grade in this area, we were not surprised to discover that this place is well below average in terms of its quantity of physical therapy and registered nurse hours provided to its residents based on the figures we assessed. The final statistic we assessed in this category is the number of patients who were able to leave the nursing home and return to the community. We discovered that just 0 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar metrics sunk this nursing home's short-term care score.
Long-term Care Quality
The next area we rated is long-term care. This nursing home received a poor grade of F in that area. In a long-term care setting, the nursing home's primary goal is to maintain residents' quality of life and keep them safe. Once we looked at the amount of care provided by nurses and other staff, we then analyzed the nursing home's vaccination records. We were optimistic to discover that this nursing home vaccinated 100 percent of its residents for pneumonia. To our surprise, this facility was able to limit hospitalizations. With only 0.02 hospitalizations per 1,000 long-term resident days, this nursing home has fewer hospitalizations than the majority of nursing homes. Unfortunately, a few of its other scores in this category were not as favorable as these.
