While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. These inspection scores take several factors into consideration, including deficiencies and federal fines. You can find more information about each of these factors by obtaining copies of nursing homes' inspection reports. Fortunately, although this facility had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This means that CMS did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Lastly, this facility was flagged by CMS for possible abuse or neglect. We prefer to avoid facilities with this designation. Please do your best to find an alternative nursing home.

We were also surprised to find that this facility received a decent nursing grade this year. We gave it a grade of C in that category. Our nursing rating analyzes the facility's level of nurse staffing. We factor in both the levels of skill of the nurses and the quantity of hours spent with residents. This place provides 2.8 hours of nursing care per patient per day. This is a lower figure than we were hoping for. Finally, nursing ratings also factor in quality-based metrics, such as preventing major falls. Despite not having the most impressive total nursing hours, this facility actually performed admirably in the area of preventing major falls. This is typically a reliable indicator that a nursing home has quality controls in place. Many falls could be avoided if a facility provides enough nurses to assist its residents.

Turning to the category of long-term care, this facility was given just a D in that category. When nursing homes receive a score in this range in this category it typically means the nursing home didn't perform well in our quality measures relating to patient care. Once we assessed the volume of care provided by nurses, we then looked at the nursing home's vaccination data. We were a bit alarmed this nursing home vaccinated a relatively low 78.15631 percent of its residents against pneumonia. Surprisingly, this place actually fared well at keeping its residents out of the hospital. It had only 0 hospitalizations per one thousand long-term resident days. This is its best feature in this category.

The final area we graded is short-term care, in which this nursing home received a poor score in this category as well. We gave it an abysmal F for this area, which is definitely a major concern. In determining these short-term care grades, we analyze the facility's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of therapists. The purpose is to create a barometer for comparing the rehabilitation services of nursing homes. As you might expect, we discovered that this facility offers significantly less physical therapist and registered nurse hours per resident than the average facility. Finally, we assessed the number of residents that ultimately returned home from this facility. This facility didn't fare well here either. In fact, we found that just 39.5 percent of this facility's residents were able to return home.
