Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection grades take several factors into consideration, including deficiencies and federal fines. You can find more information about each of these issues by obtaining copies of nursing homes' inspection reports. While this facility had a few minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are those found in categories G, H, I, J, K and L. This means that CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies aren't the end of the world.
Long-term Care Quality
Sadly, this nursing home only earned a grade of D for its long-term care rating, which was actually one of its more respectable category grades. Facilities that do not score well in long-term care often do not provide as much nursing care and also may be struggling in a few of the areas of routine medical care we assessed. On top of looking at the quantity of care provided by aids and other staff, we also looked at the percentage of residents vaccinated against pneumonia. We found that this facility gave the vaccine to only 88.92618 percent of its patients, which is several points lower than we expected. Surprisingly, this place actually fared well at keeping its patients out of the hospital. In fact, it had just 0.03 hospitalizations per 1,000 long-term resident days. This was its best score in this category.

Nurse Quality
Among this nursing home's several poor grades was its F in the area of nursing care. We looked at the licensure of nurses working for the facility, in addition to the quantity of time the nurses spent with residents, in calculating our score in this category. This nursing home offered only 0.2 hours of nursing care per resident on a daily basis. This is an alarmingly low total. Lastly, we also assessed some nursing quality measures in computing our nursing scores. This facility fared well in terms of avoiding pressure ulcers and major falls. We consider these statistics to be reliable measures of the quality of nursing care.
Short-term Care Quality
The final area we rated is short-term care. This facility was given a poor grade of F in that area. In calculating our short-term care grades, we size up the facility's levels of skilled nursing, which includes registered nurses, physical therapists respiratory therapists and other licensed professionals. This area is typically a useful assessment of the nursing home's rehabilitation services. Not surprisingly, we found that this nursing home provides substantially less physical therapist and registered nurse hours per patient than the average nursing home. The final statistic we looked at in this category is the number of patients that returned home from the nursing home. This nursing home struggled quite a bit in this metric as well, with just 4.7 percent of its residents returning home. This was well below average.
