Facility Inspections
While we graded this nursing home incredibly poorly overall, it earned a very impressive inspection report this year. As a result, it earned one of our more favorable scores in that area with a B. Perhaps the most critical factor we consider in calculating our inspection grades is deficiencies. Deficiencies can be found on a facility's inspection reports. It is typically in your best interest to avoid facilities that have a long list of severe deficiencies. While this facility had a few deficiencies on its government inspection report, none were major deficiencies based on CMS' scale. A few minor deficiencies aren't the end of the world.
Short-term Care Quality
Sadly, this facility only earned a D for its short-term care score, which is actually one of its higher category grades. In the area of short-term care, we try to qualify indicators of a facility's rehabilitation. We analyze the facility's skilled nursing services, such as those performed by registered nurses and physical therapists. Based on its abysmal grade in this area, we weren't shocked to find this nursing home offers far fewer physical therapist and registered nurse hours per patient than most other facilities. Lastly, we looked at the percentage of patients who were able to return to the community from this facility. We found that just 4.6 percent of this facility's patients returned home.

Nurse Quality
Unfortunately, we gave this facility an F for its nursing grade. The nursing rating consists of a handful of components, however, the main consideration is the number of nurse hours per patient per week. This facility offered only 0.4 hours of nursing care per patient daily. This is not a very impressive figure. Finally, we also assessed some nursing quality-based metrics in calculating our nursing grades. This facility fared well in terms of avoiding major falls and pressure ulcers. We consider these metrics to be indicators of the quality of nursing care.
Long-term Care Quality
The final category we rated is long-term care. Unfortunately, we gave it an abysmal F for this area, which is a poor grade. When facilities receive a score in this range in long-term care it typically means the facility did not perform well in our measures relating to resident care. In addition to looking at the volume of care provided by nurses aids and other staff, we looked at the number of residents vaccinated for pneumonia. Frankly, we were a bit alarmed this facility vaccinated a relatively low 90.56604 percent of its residents. To our surprise, this place was actually decent at keeping its residents out of the hospital. In fact, it had only 0 hospitalizations per 1,000 long-term resident days. This was its best feature in this category.
