Facility Inspections
B-
Historical
While this nursing home's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a B- for that category, which is one of our better scores. We would argue that one of the most important factors in assessing an inspection report is deficiencies. We especially look at the severity of the deficiencies. Fortunately, although this nursing home had some minor dings on its government inspection report, it had no severe deficiencies. Severe deficiencies are ones found in categories G, H, I, J, K and L. This tells you the government inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Long-term Care Quality
B-
Historical
We were also stunned to learn that this facility earned a reasonably solid long-term care score in our assessment. We awarded it a B- in this category. If you are looking for anything other than short-term rehabilitation, you should take a close look at long-term care ratings. Once we concluded our assessment of the quantity of care provided by nurses, we next considered the facility's vaccination statistics. We were pleased to learn that this facility vaccinated 97.262245 percent of its patients against pneumonia. Lastly, this place was able to limit hospitalizations. With just 1.38 hospitalizations per 1,000 long-term resident days, this facility had fewer hospitalizations than many nursing homes.

Nurse Quality
C
Historical
This nursing home also didn't perform terribly in the area of nursing. In fact, we awarded it a grade of C for that category, which is another one its better scores. Our nursing grade analyzes the nursing home's nurse staffing levels. We factor in both the levels of training of those nurses and the number of hours spent with residents. This nursing home provides 3.6 hours of nursing care per resident per day. Lastly, our nursing grades also factor in some quality-focused statistics. Specifically, we look at the percent of residents who sustained pressure ulcers and major falls. We look at these statistics as predictive measures of the quality of nursing care, since better care tends to reduce these problems.
Short-term Care Quality
D
Historical
The last area we assessed was short-term care, which proved to be this nursing home's weakest area. We awarded this facility a D in that area. Our short-term care grade is often employed to measure a facility's rehabilitation services. To provide high-end rehabilitation services, facilities generally must have better levels of highly skilled nursing. Skilled nursing includes registered nurses, physical therapists, speech therapists and other licensed professionals. Based on its weak score in this area, we were not shocked to learn this facility provides substantially fewer registered nurse and physical therapist hours per resident than most other nursing homes. Finally, we looked at the percentage of residents who ultimately were able to return home from this facility. This nursing home didn't fare well here either. In fact, we found that just 31.8 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.
