Facility Inspections
While we gave this facility a bad overall rating, we actually awarded it a somewhat respectable grade in the area of inspections, where we gave it a B-. This was better than its other category grades. These inspection grades weigh several factors, including deficiencies and federal fines. You can learn more about each of these items by reviewing copies of nursing homes' inspection reports. Although this nursing home had some deficiencies on its report, none of them were major deficiencies based on CMS' scale. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Nurse Quality
We also ended up giving this facility a fairly respectable grade in the area of nursing, where it received a grade of B-. When calculating our nursing ratings, we look at both staffing levels and the levels of licensure of those nurses. This nursing home provides 0.5 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Finally, nursing ratings also factor in quality-based metrics, such as preventing major falls. Despite not having the most impressive total nursing hours per resident, this facility actually performed admirably in the area of preventing major falls. This is generally a good indicator that a facility has sufficient quality controls in place. Some falls could be avoided if a facility provides enough nurses aids to assist its residents.

Short-term Care Quality
The third most favorable area we awarded this nursing home in any category came in the area of short-term care, where we awarded it a grade of B-. Our short-term care scores are based in part on a facility's quantity of skilled highly skilled professionals. This includes a vast range of nursing services, ranging from registered nurses to physical and respiratory therapists, in addition to other types of therapy. Unfortunately, we found that this facility provided fewer physical therapist and registered nurse hours per resident than most other nursing homes. The more important thing is determining what bearing this had on the quality of care. The last datapoint we considered in this category is the percentage of residents that ultimately were able to return home from the facility. We found that just 6.2 percent of this facility's residents returned home.
Long-term Care Quality
Moving on to our last area, we awarded this nursing home a grade of D for our long-term care category. This is the nursing home's least impressive category score. Facilities that do not fare well in long-term care typically aren't as well-staffed and are lagging in a few of the areas of routine personal care we assessed. After assessing the amount of care provided by nurses aids and other staff, we turned to the nursing home's vaccination record. Candidly, we were a bit worried that this nursing home vaccinated 65.89404 percent of its residents for pneumonia, which is quite a few points less than what we expected. To our surprise, this place was actually decent at keeping its residents out of the hospital. It had only 0.01 hospitalizations per 1,000 long-term resident days. This is its best score in this category.
