Facility Inspections

Grade: A

Although we didn't rate this facility favorably overall, we did give it an excellent score in the category of inspections, where it earned an A. Our inspection scores are based on many datapoints located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these inspections. You should especially avoid nursing homes that have severe deficiencies linked to risks to resident well being. While this facility had some deficiencies on its report, none of them were severe based on CMS' scale. A few minor deficiencies are not the end of the world.

Long-term Care Quality

Grade: C

This facility was awarded its second best category grade in the area of long-term care. We gave this nursing home a grade of C in this area, which is a respectable score. Our long-term care ratings are important for individuals looking for non-healthcare services such as personal care. Once we concluded our assessment of the volume of care provided by nurses, we next considered the nursing home's vaccination statistics. We were optimistic to find that this facility vaccinates 99.48718 percent of its residents against pneumonia. This facility was able to keep its residents out of the hospital. It had only 0.02 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.

Short-term Care Quality

Grade: D

This facility's second least favorable category is short-term care, where it earned just a D in that area. This is a relatively poor score. With our short-term care score, we endeavor to forge a fair measure for rehabilitation services. In doing so, we look at a facility's scope of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. Based on its score in this category, we weren't surprised to learn that this place was well below average in terms of its number of physical therapy and registered nurse hours offered to its patients based on the measures we assessed. Frankly, this was what we expected to find. Finally, we considered the percentage of patients who were able to return to the community from this nursing home. This definitely was not a strength for this nursing home. We found that just 4.7 percent of this facility's patients returned home. At most facilities, about half of their residents are able to return home so this is well below average.

Nurse Quality

Grade: F

The next area we analyzed was nursing, where this facility received a poor score in this category also. Sadly, it received a lowly F in this area, which is obviously somewhat alarming. We weighed the levels of nurses working for the nursing home, as well as the quantity of time the nurses were with patients, in calculating our score in this area. With only 0.3 hours of nursing care per patient each day, this place's nurse staffing levels were well below the national average. Lastly, we also assessed some nursing quality-based metrics in determining our nursing grades. This nursing home fared well in the area of minimizing its residents' major falls and pressure ulcers. Many consider these data points to be good measures of the quality of nursing care.

Century Villa Health Care & Rehabilitation Quality Metrics

Minimizes Pressure Ulcers

Grade: B

In Century Villa Health Care & Rehabilitation, 8% of Patients had Pressure Ulcers

This indicates the percentage of patients that sustained a pressure ulcer. Pressure ulcers, also called bed sores, are routinely the result of patients not being moved frequently enough. Better nursing protocols limits the percentage of residents in a nursing home who sustain pressure ulcers.

Minimizes Serious Falls

Grade: D

In Century Villa Health Care & Rehabilitation, 6.15% of Patients had Serious Falls

This indicates the percentage of patients who suffered from a major fall. Falls resulting in severe injury are routinely linked to poor nursing care. Closer supervision can minimize the percentage of residents in a nursing home who sustain major falls.

Minimizes Urinary Tract Infections

Grade: B

In Century Villa Health Care & Rehabilitation, 1.57% of Patients had UTIs

This statistic gauges the percent of long-term stay residents who have suffered from a UTI. While more of these infections could reflect poorly on a facility's nursing care, it is problematic to compare different facilities due to inconsistent reporting standards.

Appropriately Uses Anti-Psychotic Medication

Grade: B

In Century Villa Health Care & Rehabilitation, 14.43% of Patients use Anti-Psychotic Medication

This metric indicates the percentage of long-term care patients that were administered antipsychotic drugs. Excessive reliance on these medications may suggest a nursing home is using these medications to control patient behavior in scenarios where such drugs are not medically indicated. Nevertheless, some facilities may need to rely on these drugs due to an increased number of residents suffering from cognitive disorders.

Appropriately Uses Anti-Anxiety Medication

Grade: B-minus

In Century Villa Health Care & Rehabilitation, 21.02% of Patients use Anti-Anxiety Medication

This is the percent of patients given antianxiety medications. Antianxiety drugs are given to patients experiencing anxiety or depression.

This is a measure of the percent of long-term care residents who needed increased assistance with activities of daily living over time. ADL's include activities such as moving around and bathing. Some would argue that this is a reasonable measure of a patient's well-being.

Ability to Keep Residents Mobile

Grade: B-minus

In Century Villa Health Care & Rehabilitation, 12.68% of Residents

This indicates the percent of patients that remained mobile levels over time. Optimizing mobility is often a great sign for residents' health.

Hospitalizations

Grade: D

In Century Villa Health Care & Rehabilitation, 0.02 Hospitalizations per 1,000 resident days

This is a measure of the number of times residents are hospitalized per 1,000 days of patient care.

Short-term Care: Rehospitalizations

Grade: B-plus

In Century Villa Health Care & Rehabilitation, 12.68% of Residents Rehospitalized

This metric measures the number of times residents are rehospitalized per 1,000 days of short-term resident care.

Short-term Care: ER Visits

Grade: B-minus

In Century Villa Health Care & Rehabilitation, 13.86% of Patients

Measures the number of times residents are sent to the emergency room per thousand days of short-term patient care. Avoiding medical emergencies is one way to assess patient care during rehabilitation.

Short-term Care: Facilitates Functional Improvement

Grade: A

In Century Villa Health Care & Rehabilitation, 88.06% of Resident

This tells you the percentage of short-term stay patients that saw functional improvements, such as enhanced ability to perform activities of daily living. High levels of independence with activities of daily living often correlates with superior rehabilitation.
