Facility Inspections
Although we were not high on this facility's overall performance, it did receive an impressive inspection report this year with few dings from the applicable government inspectors. As a result, it received one of our better grades in that category with a B. We weigh several aspects of a nursing home's inspection report in computing our inspection ratings. One of those factors is health deficiencies. You should keep in mind that the severity of these deficiencies is usually more meaningful than the number of deficiencies, as some end up being relatively insignificant. Although this nursing home had a few deficiencies on its government inspection report, none of them were major deficiencies based on CMS' deficiency scale. A couple minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
This nursing home received its next highest category grade in the category of long-term care. We awarded this facility a grade of B- in this area, which is a respectable grade. In our long-term care grade, we look at the sheer volume of services offered by a facility, as well as a few qualitative measures. On top of looking at the amount of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. Fortunately, this facility vaccinated 97.44898 percent of its residents, which is a very respectable total. Lastly, this facility was also able to limit hospitalizations. It had less than one hospitalization per one thousand long-term resident days, which is a remarkably low number.

Nurse Quality
We were surprised to find that this facility also excelled in the category of nursing. We awarded it an impressive grade of D for that category, which is one of our best scores. There are a host of subcategories within this grade. Most of these factors relate to levels of nurse staffing. With just 0.3 hours of nursing care per resident per day, this nursing home's nurse staffing levels were well below average. Lastly, we also looked at some nursing quality measures in calculating our nursing scores. This facility performed well when it comes to minimizing its residents' major falls and pressure ulcers.
Short-term Care Quality
The last area we rated was short-term care, which proved to be a low point for this facility. We gave this nursing home a D in short-term care. In computing these short-term care grades, we look at the facility's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of therapists. Our purpose is to create a tool for comparing the rehabilitation services of various facilities. Based on its grade in this category, we were not surprised to find that this facility was well below average in terms of the number of physical therapy and registered nurse hours offered to its residents based on the measures we looked at. Lastly, we considered the percentage of patients who returned home from this facility. This facility didn't fare well here either. In fact, we found that just 0 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.
